A · “I add extra salt to my food so it tastes better.”

A · Stop the oxytocin infusion

Late decelerations point to reduced blood flow through the placenta. Stopping oxytocin reduces contractions; the nurse then turns the patient onto her side, gives IV fluids as prescribed and informs the provider.

A · Stop the oxytocin infusion

B · Stop the infusion straight away and follow the extravasation protocol

Suspected extravasation of a vesicant needs the infusion stopped immediately. Local protocols then guide aspiration through the cannula, specific treatments, elevation, documentation and specialist review.

B · Stop the infusion straight away and follow the extravasation protocol

D · Not start until a correct identity band is in place and identity is confirmed

Positive patient identification, using an identity band checked against the blood unit and prescription, is essential before every transfusion. Wrong blood incidents can cause fatal ABO incompatibility reactions.

D · Not start until a correct identity band is in place and identity is confirmed

D · At the bedside, in the patient's presence, using two identifiers

Specimens are labeled immediately after collection, at the bedside, after checking two patient identifiers such as name and date of birth. Pre-labeling or labeling away from the patient can put the wrong patient's name on the sample, which can lead to harmful decisions.

D · At the bedside, in the patient's presence, using two identifiers

A · It raises the risk of acute pancreatitis

Triglycerides are monitored in patients receiving lipid emulsions, because high levels can cause pancreatitis and show that lipid is not being cleared. The result is reported, and the lipid dose may be reduced or held. Triglycerides Test · MedlinePlus (NIH) (2024)

A · It raises the risk of acute pancreatitis

D · Pull the pinna down and back

In children under about 3 years, the pinna is pulled down and back; in adults it is pulled up and back. The child then lies with the treated ear up for a few minutes.

D · Pull the pinna down and back

B · It is a partial agonist at dopamine D2 receptors

Aripiprazole is a D2 partial agonist, reducing dopamine activity where it is high and providing some where it is low. It causes less weight gain and prolactin rise than many antipsychotics, but akathisia is common.

B · It is a partial agonist at dopamine D2 receptors

B · Bleeding, including a sudden change in neurological status

The main risk of thrombolysis is bleeding, from puncture sites, gums or the gut, and most seriously intracranial haemorrhage. The nurse monitors puncture sites, vital signs and neurological status and minimises invasive procedures. Reperfusion arrhythmias are also common.

B · Bleeding, including a sudden change in neurological status

A · Pneumothorax

Pneumothorax is a common complication of thoracentesis, caused by the needle puncturing the lung or air entering the chest. The nurse assesses the patient, gives oxygen and informs the team, who will arrange a chest X-ray. An effusion re-accumulates over days, and infection appears later.

A · Pneumothorax

A · Low pH and low bicarbonate

Failing kidneys cannot excrete acid or regenerate bicarbonate, causing a metabolic acidosis with low pH and low bicarbonate. The patient may breathe faster to blow off CO2 as compensation.

A · Low pH and low bicarbonate

B · Carbamazepine can cause hyponatraemia through an SIADH-like effect

Carbamazepine (and oxcarbazepine) can increase the effect of ADH, leading to water retention and hyponatraemia. Severe hyponatraemia causes confusion and seizures. The drug and sodium need urgent medical review.

B · Carbamazepine can cause hyponatraemia through an SIADH-like effect

C · A non-smoking woman with motion sickness who will need opioids

Apfel risk factors are female sex, non-smoking status, a history of PONV or motion sickness, and postoperative opioids. Patients with more factors receive more preventive antiemetics.

C · A non-smoking woman with motion sickness who will need opioids

D · A vulvar or vaginal haematoma

A haematoma causes severe pain and pressure out of proportion to the birth, often with a firm uterus and normal lochia. Signs of hidden blood loss such as tachycardia must be reported.

D · A vulvar or vaginal haematoma

B · Uterine inversion

An absent fundus on palpation with a mass in the vagina and shock suggest uterine inversion. Help is called immediately, oxytocin is stopped and the uterus is replaced as soon as possible by the medical team.

B · Uterine inversion

B · Clean intermittent catheterisation

B · Keep a safe distance, call for help and avoid trying to take the chair alone

الإجابة: h = 76,4 in

h = 194/2,54 = 76,38 ≈ 76,4 in

h = 76,4 in

c = 119/88,4 = 1,346 ≈ 1,35 mg/dL

c = 1,35 mg/dL

الإجابة: V = 5 mL

Solution strengths: a percentage strength is grams per 100 mL, so 1% = 1 g/100 mL = 10 mg/mL. A ratio strength 1:1000 means 1 g in 1000 mL = 1 mg/mL; 1:10 000 is 0,1 mg/mL. For a dilution the amount of drug stays the same: C₁·V₁ = C₂·V₂.

5% = 50 mg/mL

V = 250/50 = 5 mL

V = 5 mL

12,5·20 = 250 mg

V = (250/100)·5 = 12,5 mL

V = 12,5 mL

R = 25 gtt/min

10·60 = 600 min

R = 250·60/600 = 25 gtt/min

R = 25 gtt/min

Infusion rates: a pump is set in mL/h = volume/time in hours. A gravity drip is counted in drops per minute: gtt/min = volume (mL) × drop factor (gtt/mL)/time (min). The drop factor is printed on the tubing package: macrodrip sets deliver 10, 15 or 20 gtt/mL (blood sets often 10 gtt/mL in the US, 15 in the UK) and microdrip sets 60 gtt/mL. Always use the drop factor the question or package gives.

500/6 = 83,33 mL/h

R = 210/2 = 105 mL/h

Δ = (105 − 83,33)/83,33·100% = 26%

Δ = 26%

U = 1200/(73·8) = 2,055 ≈ 2,05 mL/kg/h

U = 2,05 mL/kg/h

06:15 → 08:45 = 2,5 h

150·2,5 = 375 mL

V = 500 − 375 = 125 mL

V = 125 mL
