C · Call emergency services now and note the time it started

Stroke is time-critical. Emergency services can begin assessment on the way and take the patient to a stroke center; the time of onset guides treatment.

C · Call emergency services now and note the time it started

B · New atrial fibrillation, heart rate 150/min, blood pressure 86/50 mmHg, dizzy

Rapid atrial fibrillation with hypotension and dizziness means the heart is not pumping enough blood; the patient is unstable. The other patients are within target or have predictable needs.

B · New atrial fibrillation, heart rate 150/min, blood pressure 86/50 mmHg, dizzy

A · Respiratory rate 24/min, new confusion, systolic BP 95 mmHg

qSOFA scores one point each for a respiratory rate of 22/min or more, altered mental status and a systolic BP of 100 mmHg or less. Two or more points mean higher risk, so the first patient needs urgent escalation.

A · Respiratory rate 24/min, new confusion, systolic BP 95 mmHg

D · An aplastic crisis, often caused by parvovirus B19

Patients with sickle cell disease depend on a high reticulocyte count because their red cells live only a short time. Parvovirus B19 can stop red cell production for days, causing a rapid fall in hemoglobin with very low reticulocytes. Transfusion may be needed.

D · An aplastic crisis, often caused by parvovirus B19

B · Unopened insulin is kept in the fridge and must not be frozen

Unopened insulin is refrigerated; in-use pens or vials are usually kept at room temperature for a limited time stated by the manufacturer. Frozen or overheated insulin must be discarded.

B · Unopened insulin is kept in the fridge and must not be frozen

C · A change in level of consciousness

Subtle changes in consciousness, such as restlessness, confusion or a falling GCS, are often the first signs. Pupil changes and Cushing's triad (hypertension, bradycardia, irregular breathing) are late signs.

C · A change in level of consciousness

A · Potassium and creatinine

ACE inhibitors lower aldosterone, so potassium can rise, and they reduce glomerular filtration pressure, so creatinine can rise, especially with renal artery stenosis or dehydration. Baseline and early follow-up urea, electrolytes and creatinine are standard.

A · Potassium and creatinine

A · A large difference in blood pressure between the arms

A difference of more than about 20 mmHg in systolic pressure between the arms, a pulse deficit or a new aortic regurgitation murmur suggests dissection. Sweating, tachycardia and rest pain also occur in MI. Thrombolysis and anticoagulants could be disastrous in dissection, so the distinction matters.

A · A large difference in blood pressure between the arms

A · Loss of movement and pain sense, with position sense kept

Anterior cord syndrome usually follows damage to the anterior spinal artery. Motor, pain and temperature pathways are lost below the injury, but the dorsal columns (fine touch, vibration and proprioception) at the back are spared.

A · Loss of movement and pain sense, with position sense kept

C · Give IV antibiotics at once and start sepsis treatment

Meningococcal septicaemia can kill within hours. Antibiotics must not be delayed for a lumbar puncture or scan. Fluids and other sepsis care are started, and a lumbar puncture is often unsafe in a shocked patient.

C · Give IV antibiotics at once and start sepsis treatment

D · Necrotising fasciitis

Necrotising soft tissue infection spreads rapidly along fascial planes and can cause septic shock. It is a surgical emergency needing urgent debridement, IV antibiotics and sepsis care.

D · Necrotising fasciitis
