D · A patient with a sealed radioactive implant for cervical cancer

A sealed radioactive implant (brachytherapy) emits radiation, and pregnant staff should not care for these patients. The other patients pose no special risk to a pregnancy.

D · A patient with a sealed radioactive implant for cervical cancer

B · A possible myocardial infarction that needs urgent action

Chest pain that lasts and is not relieved by rest or nitroglycerin suggests acute coronary syndrome, possibly a heart attack. The nurse acts at once: ECG, escalation and prescribed treatment.

B · A possible myocardial infarction that needs urgent action

B · Open the wrapper away from the field and drop the item on it without reaching over

The nurse opens the package so the sterile item can be dropped onto the field from a short height, without the wrapper or arms crossing over the sterile area. The item must land well inside the non-sterile border.

B · Open the wrapper away from the field and drop the item on it without reaching over

A · Explain the risks, agree on ways to reduce them, respect the decision and document

A competent patient can accept a risk. The nurse explains the danger, offers measures such as a low bed, non-slip footwear, a clear path and a call light, respects the choice and documents the discussion.

A · Explain the risks, agree on ways to reduce them, respect the decision and document

A · Blood culture bottles

Blood cultures are drawn first to reduce contamination. A standard order of draw then follows (citrate, serum, heparin, EDTA, fluoride), which prevents additives being carried from one tube into the next and distorting results.

A · Blood culture bottles

A · Sodium 119 mEq/L in a patient with new confusion and vomiting

Severe hyponatremia with confusion and vomiting suggests cerebral edema and can progress to seizures and coma. The other results are mildly abnormal and expected for those conditions, so they are less urgent. Low blood sodium - Medical Encyclopedia · MedlinePlus (NIH) (2025); Sodium Blood Test · MedlinePlus (NIH) (2024)

A · Sodium 119 mEq/L in a patient with new confusion and vomiting

A · البوتاسيوم والفوسفات

D · IV fluid rehydration, followed by a bisphosphonate

Hypercalcaemia of malignancy causes dehydration, so IV fluids are given first, followed by a bisphosphonate to reduce bone resorption. Calcium, renal function and fluid balance are monitored.

D · IV fluid rehydration, followed by a bisphosphonate

C · To prevent dehydration and keep secretions thinner

Fever, sweating and tachypnoea increase insensible fluid loss. Good hydration prevents dehydration and helps keep secretions less sticky. Patients with heart failure or other restrictions need individual plans.

C · To prevent dehydration and keep secretions thinner

C · An intravenous beta-blocker

In acute dissection, IV beta-blockers are usually given first to reduce heart rate and the rate of rise of pressure, limiting extension of the tear. Vasodilators may follow once the heart rate is controlled. Antiplatelets, anticoagulants and thrombolytics are avoided. Type A dissection needs urgent surgery.

C · An intravenous beta-blocker

D · The bladder is not emptying properly

A large post-void residual means incomplete emptying, for example from obstruction or a weak detrusor. It raises the risk of infection and overflow incontinence and may need intermittent catheterisation or treatment of the cause.

D · The bladder is not emptying properly

C · Explain that this is emotional lability after stroke and help the family understand

Emotional lability (pseudobulbar affect) is common after stroke: crying or laughter does not match the person's mood. Explaining it, staying calm and gently distracting the patient helps. True depression is also common and should be screened for separately.

C · Explain that this is emotional lability after stroke and help the family understand

B · A moist, not wet, wound bed helps cells move and heal faster

A moist environment supports cell migration, autolytic debridement and less pain. Too much moisture causes maceration, so dressings are chosen to balance exudate.

B · A moist, not wet, wound bed helps cells move and heal faster
