1Formule e metodi

Infermieristica di salute mentale

Metodo

A patient taking azathioprine is prescribed allopurinol for gout. What is the main danger?

  1. AGout becomes worse because allopurinol is inactivated by azathioprine
  2. BLiver enzyme induction, making azathioprine ineffective

3Soluzioni

1.Infermieristica di salute mentale

Risposta: A · Keep to his usual routines where possible and prepare him for any changes

Perché è questa la risposta

Predictability reduces distress. Nurses keep routines, use clear and concrete language, reduce sensory stimulation, use visual supports and work closely with the parents, who know the child best.

Risposta

A · Keep to his usual routines where possible and prepare him for any changes
2.Infermieristica di salute mentale

Risposta: D · Discuss the problem openly and involve patients in finding a solution

Perché è questa la risposta

In a therapeutic milieu, the ward environment itself is used for treatment. Shared decision-making in community meetings builds responsibility, social skills and trust.

Risposta

D · Discuss the problem openly and involve patients in finding a solution
3.Psicologia clinica

Risposta: D · Formazione reattiva

5.Farmacologia

Risposta: C · Kidney and liver clearance often decline, and sensitivity to drugs increases

Perché è questa la risposta

Ageing reduces kidney function, changes body composition and often slows liver metabolism. Many drugs also have stronger effects on the ageing brain, so doses are started low and increased slowly.

Risposta

C · Kidney and liver clearance often decline, and sensitivity to drugs increases
6.Farmacologia

Risposta: D · Severe bone marrow suppression, because azathioprine breakdown is reduced

Perché è questa la risposta

Azathioprine is converted to mercaptopurine, which is partly inactivated by xanthine oxidase. Allopurinol blocks this enzyme, so levels rise and dangerous myelosuppression can occur unless the dose is greatly reduced or the combination avoided.

Risposta

D · Severe bone marrow suppression, because azathioprine breakdown is reduced
