AAccompanying a stable patient to the dining room and recording his food intake
BEvaluating whether a PRN medicine given an hour ago has worked
CTeaching a patient about the side effects of lithium
DCarrying out the full admission suicide risk assessment of a newly admitted patient
3.Psicologia clinicaMedia
5.FarmacologiaMedia
A patient takes low-dose weekly methotrexate for rheumatoid arthritis. Which monitoring is needed?
ADaily blood glucose checks
BWeekly INR checks and a monthly ECG to check the heart rhythm
CMonthly hearing tests
DRegular blood count, liver and kidney function tests
6.FarmacologiaDifficile
A patient with diabetic ketoacidosis is about to start an IV insulin infusion. Serum potassium is 3,1 mmol/L (mEq/L). Why is this a concern?
AInsulin raises potassium further and can cause cardiac arrest, so the infusion is given faster
BInsulin moves potassium into cells, so the level can fall dangerously low
CLow potassium makes insulin ineffective, so a higher dose is needed
DPotassium has no link with insulin, so the infusion is started as planned
3Soluzioni
1.Infermieristica di salute mentale
Risposta: D · Stay with him in a calm, quiet place and reassure him that the effects are from the drug and will pass
Perché è questa la risposta
For a frightening hallucinogen experience, a calm setting, a supportive person and simple reassurance usually help. The nurse also monitors vital signs and watches for injury or other substances.
Risposta
D · Stay with him in a calm, quiet place and reassure him that the effects are from the drug and will pass
2.Infermieristica di salute mentale
Risposta: A · Accompanying a stable patient to the dining room and recording his food intake
Perché è questa la risposta
Routine, predictable tasks with stable patients, such as escorting and recording intake, can be delegated. Assessment, care planning, teaching and evaluation require the nurse's judgement.
Risposta
A · Accompanying a stable patient to the dining room and recording his food intake
3.Psicologia clinica
5.Farmacologia
Risposta: D · Regular blood count, liver and kidney function tests
Perché è questa la risposta
Methotrexate can cause bone marrow suppression and liver damage, and poor kidney function raises its levels. Blood tests are regular, alcohol is limited and folic acid is usually given on another day.
Risposta
D · Regular blood count, liver and kidney function tests
6.Farmacologia
Risposta: B · Insulin moves potassium into cells, so the level can fall dangerously low
Perché è questa la risposta
Insulin drives potassium into cells, so a low starting level can become life-threatening hypokalaemia with arrhythmias. Potassium is replaced and monitored closely; the 2024 international consensus (ADA and others) delays insulin until potassium is above 3,5 mmol/L, while UK JBDS guidance adds potassium to the fluids and asks for senior review.
Risposta
B · Insulin moves potassium into cells, so the level can fall dangerously low
