1Formule e metodi

Infermieristica di salute mentale

Metodo

A patient taking azathioprine is prescribed allopurinol for gout. What is the main danger?

  1. AGout becomes worse because allopurinol is inactivated by azathioprine
  2. BSevere bone marrow suppression, because azathioprine breakdown is reduced
  3. CLiver enzyme induction, making azathioprine ineffective
  4. DSevere hypertension from the combination

3Soluzioni

1.Infermieristica di salute mentale

Risposta: C · Let the tablet dissolve completely under the tongue without chewing or swallowing it

Perché è questa la risposta

Buprenorphine is poorly absorbed if swallowed, so it is dissolved under the tongue. Naloxone is added to discourage injection, because it is poorly absorbed sublingually but active if injected.

Risposta

C · Let the tablet dissolve completely under the tongue without chewing or swallowing it
2.Infermieristica di salute mentale

Risposta: D · A newly admitted man with depression who says he has given away his dog and written letters to his children

Perché è questa la risposta

Giving away pets or possessions and writing goodbye letters are warning signs of a suicide plan. This patient needs immediate assessment; the others have needs that can safely wait a little.

Risposta

D · A newly admitted man with depression who says he has given away his dog and written letters to his children
3.Psicologia clinica

Risposta: A · Le convinzioni (beliefs) sull'evento attivante

Risposta

A · Le convinzioni (beliefs) sull'evento attivante
4.Psicologia clinica

5.Farmacologia

Risposta: A · When started within 48 hours of the first symptoms

Perché è questa la risposta

Neuraminidase inhibitors work best early, while the virus is multiplying. Started within 48 hours, they shorten illness; in high-risk or hospitalised patients they may still be given later.

Risposta

A · When started within 48 hours of the first symptoms
6.Farmacologia

Risposta: B · Severe bone marrow suppression, because azathioprine breakdown is reduced

Perché è questa la risposta

Azathioprine is converted to mercaptopurine, which is partly inactivated by xanthine oxidase. Allopurinol blocks this enzyme, so levels rise and dangerous myelosuppression can occur unless the dose is greatly reduced or the combination avoided.

Risposta

B · Severe bone marrow suppression, because azathioprine breakdown is reduced
