5.FarmacologiaMedia
6.FarmacologiaDifficile
A patient with severe liver cirrhosis takes oral propranolol, a drug with a high first-pass metabolism. What effect is expected?
AMore drug reaches the circulation, so a lower dose may be needed
BLess drug is absorbed from the gut, so a higher dose is needed
CThe drug is excreted faster by the kidney, so it works for a shorter time
DThe first-pass effect increases, so the drug becomes inactive

1.Infermieristica di salute mentale
Risposta: A · They give rapid relief but are generally used only short-term because of tolerance and dependence
Perché è questa la risposta
Benzodiazepines act quickly by enhancing GABA, but long-term use leads to tolerance, dependence and withdrawal. SSRIs and psychological therapy are first-line long-term treatments for most anxiety disorders.
Risposta
A · They give rapid relief but are generally used only short-term because of tolerance and dependence
2.Infermieristica di salute mentale
Risposta: A · Acknowledge her strain and discuss support such as respite care and carers' groups
Perché è questa la risposta
Caregiver strain is common and can affect both the carer and the patient. Recognising it, and offering respite, practical support and carers' groups, helps the carer continue safely.
Risposta
A · Acknowledge her strain and discuss support such as respite care and carers' groups
3.Psicologia clinica
Risposta: C · Spostamento
Risposta
6.Farmacologia
Risposta: A · More drug reaches the circulation, so a lower dose may be needed
Perché è questa la risposta
In cirrhosis, less of the oral dose is removed by the liver on its first pass, partly because blood bypasses the liver through shunts. Bioavailability and plasma levels rise, so doses of such drugs often need to be reduced.
Risposta
A · More drug reaches the circulation, so a lower dose may be needed
