A toddler swallowed many of his mother's iron tablets. He has vomiting and bloody diarrhoea. Which drug may be used to bind iron?
A · "Tell me more about what that feeling is like for you."
Exploring asks the patient to describe an experience in more detail. Generalising, disapproving and changing the subject block the patient from talking.
A · Carry out a full suicide risk assessment, as with any patient who expresses suicidal intent
People with borderline personality disorder have a significantly raised risk of suicide. Every statement of suicidal intent must be assessed; labelling it as attention-seeking can lead to missed risk.
B · About 67 drops per minute
100 mL × 20 drops/mL ÷ 30 min = 66.7, so about 67 drops per minute. Gravity infusions are counted for a full minute and checked regularly.
D · Deferoxamine (desferrioxamine)
Iron poisoning can cause GI bleeding, shock, metabolic acidosis and liver failure. Deferoxamine chelates iron and is used in serious poisoning; activated charcoal does not bind iron.
