A patient on lisinopril for hypertension starts taking ibuprofen every day for knee pain. What is the main concern?
D · The member whose symptoms are seen as the family's problem
Family systems theory sees the family as a unit in which each member affects the others. The "identified patient" carries the symptoms, but the therapy looks at patterns in the whole family.
A · Pancreatitis
Valproate can cause acute pancreatitis, which can be life-threatening. Severe abdominal pain with vomiting must be reported promptly so the drug can be stopped and the patient assessed.
B · An infusion reaction; stop or slow the infusion and inform the prescriber
Rapid vancomycin infusion releases histamine, causing flushing and itching of the upper body (formerly called "red man syndrome"). It is managed by stopping or slowing the infusion, and sometimes with an antihistamine.
D · Weaker blood pressure control, kidney injury and high potassium
NSAIDs block prostaglandins that dilate the afferent arteriole and promote sodium loss. With an ACE inhibitor this can reduce blood pressure control and cause kidney injury and hyperkalaemia, especially if a diuretic is also used.
