D · Turn the patient onto their side

A drowsy patient who vomits can aspirate. Turning them to the side lets vomit drain from the mouth; suction and a prescribed antiemetic follow.

答え

D · Turn the patient onto their side

D · An anticholinergic drug, such as benztropine or procyclidine, as prescribed

Acute dystonia is an early extrapyramidal effect of antipsychotics. It responds quickly to an anticholinergic drug; it can be frightening, and laryngeal spasm can threaten the airway.

答え

D · An anticholinergic drug, such as benztropine or procyclidine, as prescribed

A · A mechanical lift used by enough trained staff

For a patient who cannot bear weight or help, a mechanical lift prevents injury to both patient and staff. Lifting under the arms can injure the shoulders, and pivot transfers need weight bearing.

答え

A · A mechanical lift used by enough trained staff

A · Discard the syringe and prepare the dose again with new equipment

In aseptic non-touch technique, key parts such as syringe tips and IV hubs must not touch any non-sterile surface. A contaminated key part is not cleaned and reused. It is replaced.

答え

A · Discard the syringe and prepare the dose again with new equipment

A · Kidney function (creatinine clearance)

Dabigatran is excreted mainly by the kidneys, so poor kidney function raises the drug level and bleeding risk. Kidney function is checked before starting and regularly afterwards. Routine INR monitoring is not used for direct oral anticoagulants.

答え

A · Kidney function (creatinine clearance)

A · Primary hypothyroidism, which can raise lipids and lower sodium

A high TSH with a low free T4 is primary hypothyroidism. Low thyroid hormone reduces LDL clearance, raising cholesterol, and reduces free water excretion, which can cause hyponatremia. These often improve with levothyroxine.

答え

A · Primary hypothyroidism, which can raise lipids and lower sodium

A · Isolate with droplet precautions, test and expect early antivirals

Influenza spreads mainly by droplets and contact, so the patient is isolated with droplet precautions and tested. Antivirals such as oseltamivir work best if started early, ideally within 48 hours of symptoms, and are recommended for hospitalised and high-risk patients. Vaccination of staff and patients reduces outbreaks.

答え

A · Isolate with droplet precautions, test and expect early antivirals

C · It may turn the urine dark yellow or brown

Nitrofurantoin can make urine dark yellow or brown, which is harmless. Taking it with food reduces nausea. Patients should report breathlessness, cough or tingling, which are rare but serious effects.

答え

C · It may turn the urine dark yellow or brown
