- 糖尿病の緊急事態では、DKA(ケトン、アシドーシス)と HHS(非常に高い浸透圧、脱水)を区別し、まず輸液を行います。

C · Poor ventilation causing carbon dioxide to build up
A low pH with a high PaCO₂ and normal bicarbonate is respiratory acidosis. The patient is not breathing out enough carbon dioxide, so the priority is to support ventilation and escalate.
C · Poor ventilation causing carbon dioxide to build up
D · The RN reassesses the patient’s learning needs and adjusts the teaching
Evaluating learning and changing the teaching plan belong to the RN. The LPN/LVN can reinforce teaching, but a teaching plan that is not working needs RN reassessment.
D · The RN reassesses the patient’s learning needs and adjusts the teaching
B · Below the level of the bladder and off the floor
Keeping the bag below the bladder prevents backflow of urine into the bladder, and keeping it off the floor prevents contamination. Both reduce the risk of catheter-associated urinary tract infection.
B · Below the level of the bladder and off the floor
A · Decontaminate them outside, with staff wearing appropriate PPE
Contaminated patients are decontaminated outside the department so the chemical is not brought inside to harm staff and other patients. Staff doing this wear PPE suited to the hazard.
A · Decontaminate them outside, with staff wearing appropriate PPE
D · An unformed (liquid or soft) stool
C. difficile testing is done on unformed stool from patients with diarrhea, because many people carry the organism without disease. Testing formed stool, or stool after laxatives, can lead to misdiagnosis. C. diff Testing · MedlinePlus (NIH) (2026)
D · An unformed (liquid or soft) stool
High potassium level · MedlinePlus (NIH) (2025)
C · Night-time hypoglycaemia
Nocturnal hypoglycaemia can cause sweating, restless sleep, nightmares and morning headache. Checking glucose overnight and reviewing insulin doses can confirm and correct it.
C · Night-time hypoglycaemia
C · They reduce complications such as pneumonia, pressure ulcers and delirium
Delayed surgery and immobility increase the risk of chest infection, VTE, pressure ulcers, delirium and loss of independence. Guidelines recommend surgery within about 24–48 hours of admission (UK guidance: the day of or day after) and mobilisation soon after.
C · They reduce complications such as pneumonia, pressure ulcers and delirium
C · Chewing speeds absorption, giving a faster antiplatelet effect
Aspirin irreversibly inhibits platelet cyclo-oxygenase, reducing thromboxane A2 and platelet aggregation. Chewing a non-enteric-coated tablet speeds absorption. It reduces mortality in acute coronary syndromes and is given unless there is a true allergy.
C · Chewing speeds absorption, giving a faster antiplatelet effect
B · Stop smoking, control BP and seek help at once for sudden back pain
Small aneurysms are usually monitored with regular ultrasound, with repair considered when they reach a threshold size or grow fast. Stopping smoking slows growth, and BP and cardiovascular risk are managed. Sudden abdominal or back pain may mean rupture and needs emergency care.
B · Stop smoking, control BP and seek help at once for sudden back pain
A · A patient with a right-hemisphere stroke who is impulsive and denies having weakness
Right-hemisphere strokes often cause left-sided neglect, poor judgement, impulsivity and denial of the deficit. These patients may try to get up alone. Patients with left-hemisphere strokes are often more cautious.
A · A patient with a right-hemisphere stroke who is impulsive and denies having weakness
A · Leaky capillaries let plasma move into the tissues
Inflammatory mediators make capillaries leaky, so plasma moves into the tissues and from the burn surface. Without fluids this causes burn shock; oedema is greatest in the first day or two.
A · Leaky capillaries let plasma move into the tissues
A · A bile leak
Bile leakage from the cystic duct or a duct injury causes pain, fever and sometimes jaundice, with bile in the drain. It needs urgent surgical review and often ERCP and stenting.
