C · Dressing soaked with bright red blood and a heart rate rising to 118/min

Heavy fresh bleeding with a rising heart rate suggests hemorrhage and early shock. The other patients have routine needs that can wait.

答え

C · Dressing soaked with bright red blood and a heart rate rising to 118/min

C · Red (immediate)

In JumpSTART, a respiratory rate below 15 or above 45 per minute makes the child immediate (red). Children breathe faster than adults, so the adult cut-off of 30 is not used.

答え

C · Red (immediate)

B · Sit on the edge of the bed for a short time before standing up slowly

Changing position slowly, dangling the legs first and getting up with help give the circulation time to adjust. Rapid standing, heat and dehydration make orthostatic hypotension worse.

答え

B · Sit on the edge of the bed for a short time before standing up slowly

D · “We will put a soft bumper pad and a blanket in the crib so the baby is cozy.”

Soft bedding, bumpers and loose blankets increase the risk of suffocation and sudden infant death. The crib should hold only a firm mattress with a fitted sheet.

答え

D · “We will put a soft bumper pad and a blanket in the crib so the baby is cozy.”

D · Strenuous exercise raised CK from skeletal muscle

Strenuous exercise, IM injections and muscle trauma can raise CK from skeletal muscle. A very high CK with dark urine suggests rhabdomyolysis, and heart damage is assessed with troponin, not CK.

答え

D · Strenuous exercise raised CK from skeletal muscle

C · Gradual replacement of free water over many hours

Older adults have a reduced sense of thirst and easily develop hypernatremia from water loss. Free water is replaced gradually, because lowering sodium too quickly can cause cerebral edema. Hypertonic saline and fluid restriction would make it worse.

答え

C · Gradual replacement of free water over many hours

B · Patients can be seriously injured climbing over or becoming trapped in rails

Bed rails can cause entrapment or a fall from a greater height. Their use is based on an individual risk assessment; alternatives include a low bed, crash mats and closer observation.

答え

B · Patients can be seriously injured climbing over or becoming trapped in rails

D · The appendix has perforated, causing peritonitis

Perforation may briefly relieve the pain as pressure in the appendix falls, followed by generalised peritonitis. This needs urgent surgical review, IV fluids and antibiotics.

答え

D · The appendix has perforated, causing peritonitis

D · To improve the strength of chewing and swallowing at mealtimes

Pyridostigmine inhibits acetylcholinesterase, so more acetylcholine is available at the neuromuscular junction. Timing it before meals gives the best muscle strength for chewing and swallowing, which lowers the risk of aspiration.

答え

D · To improve the strength of chewing and swallowing at mealtimes

D · "I'll take my enzyme capsules at bedtime."

Pancreatic enzymes must be taken with meals and snacks to digest them. Alcohol must be avoided, steatorrhoea suggests under-replacement, and thirst or polyuria can signal diabetes from loss of islet cells.

答え

D · "I'll take my enzyme capsules at bedtime."
