C · Report it, because a different oxygen device is likely to be needed

Standard nasal cannulas are usually run at up to about 6 L/min; higher flows dry the nose without much benefit. If the patient still needs more oxygen, the nurse assesses and escalates for a different device.

答え

C · Report it, because a different oxygen device is likely to be needed

D · Seizure precautions and close neurological checks

Severe hyponatremia can cause seizures and falls. The nurse keeps the patient safe and reports changes; free water is restricted rather than encouraged, as prescribed.

答え

D · Seizure precautions and close neurological checks

A · Below the level of the bladder and off the floor

Keeping the bag below the bladder prevents backflow of urine into the bladder, and keeping it off the floor prevents contamination. Both reduce the risk of catheter-associated urinary tract infection.

答え

A · Below the level of the bladder and off the floor

D · “We keep the gun loaded in the nightstand, and the children know not to touch it.”

Firearms in homes with children should be stored unloaded and locked, with ammunition stored separately and locked. Telling children not to touch a gun does not reliably prevent injuries.

答え

D · “We keep the gun loaded in the nightstand, and the children know not to touch it.”

Diabetes insipidus · MedlinePlus (NIH) (2025); Urine specific gravity test · MedlinePlus (NIH) (2025); Sodium blood test · MedlinePlus (NIH) (2025)

答え

B · 尿崩症

C · Probable skin contamination of the sample

Coagulase-negative staphylococci are common skin flora. Growth in only one of several bottles, without a catheter or implant, most often reflects contamination during collection. Good skin antisepsis reduces contamination, which can lead to unnecessary antibiotics. Blood culture · MedlinePlus (NIH) (2025)

答え

C · Probable skin contamination of the sample

C · Urea (blood urea nitrogen)

Blood proteins digested in the upper GI tract are absorbed and converted to urea, so urea rises out of proportion to creatinine. Urea is part of the Glasgow-Blatchford bleeding score.

答え

C · Urea (blood urea nitrogen)

A · Malignant hyperthermia

Malignant hyperthermia is a hypermetabolic reaction triggered by volatile anaesthetics or suxamethonium (succinylcholine) in susceptible people. It is treated urgently with dantrolene, cooling and stopping the trigger.

答え

A · Malignant hyperthermia

D · Class III

NYHA class I has no limitation, class II slight limitation with ordinary activity, class III marked limitation with less than ordinary activity but comfortable at rest, and class IV symptoms at rest. The class guides treatment and prognosis.

答え

D · Class III

B · The right main bronchus

Inhaled foreign bodies most often enter the right main bronchus and right lower lobe, because the right bronchus is wider, shorter and more vertical. Signs include a unilateral wheeze, reduced breath sounds and recurrent infection. Bronchoscopy is used for removal.

答え

B · The right main bronchus

B · An oral anticoagulant

In AF the main cause of stroke is a clot from the left atrium. Oral anticoagulants such as a DOAC or warfarin prevent this far better than antiplatelets. The start date after a stroke depends on its size and bleeding risk.

答え

B · An oral anticoagulant

B · They can reduce the absorption of many other drugs

Antacids can bind or change the absorption of drugs such as some antibiotics (tetracyclines, quinolones), iron and levothyroxine. Doses are usually separated by a couple of hours.

答え

B · They can reduce the absorption of many other drugs

A · Rebound hypoglycaemia

Stopping PN suddenly can cause hypoglycaemia because insulin levels stay high for a while. Glucose is monitored, and a glucose infusion may be given according to local policy.

答え

A · Rebound hypoglycaemia
