A · Dark purple or black

A healthy stoma is moist and pink to red, and early swelling or slight bleeding is common. A dark purple or black stoma suggests poor blood supply (ischemia) and needs urgent review.

答え

A · Dark purple or black

C · Head of the bed at about 30 degrees with the neck straight

Raising the head to about 30 degrees and keeping the neck in line helps venous drainage and lowers intracranial pressure. Head-down positions and neck bending or turning can raise it.

答え

C · Head of the bed at about 30 degrees with the neck straight

A · It can mean either morphine sulfate or magnesium sulfate

“MS”, “MSO4” and “MgSO4” can be confused, and giving one drug instead of the other can cause serious harm. Drug names are written in full.

答え

A · It can mean either morphine sulfate or magnesium sulfate

D · Release the restraint at once and reposition the patient

Patients have died from strangulation when vest restraints moved up to the neck. The nurse frees the patient immediately, checks breathing and repositions them, then reassesses whether the restraint is needed at all.

答え

D · Release the restraint at once and reposition the patient

B · Every few days, until it is stable

When warfarin is started or its dose changes, the INR is checked frequently (often every few days) until it is stable in the target range. After that, the interval can be extended, often up to every 4 weeks or longer if very stable.

答え

B · Every few days, until it is stable

D · Nephrotic syndrome

Nephrotic syndrome is heavy proteinuria with low albumin, edema and high lipids. Low albumin reduces oncotic pressure, causing edema, and the liver increases lipoprotein production. There is also a higher risk of clots and infection.

答え

D · Nephrotic syndrome

D · Reorientation, glasses and hearing aids in use, and good sleep and hydration

Multicomponent prevention, including orientation, sensory aids, hydration, nutrition, mobility, sleep and avoiding unnecessary moves or drugs, reduces delirium.

答え

D · Reorientation, glasses and hearing aids in use, and good sleep and hydration

B · With all meals and snacks that contain fat

In CF, thick secretions block the pancreatic ducts, causing fat malabsorption and fatty stools. Enzyme capsules are taken with every meal and fat-containing snack, with the dose adjusted to fat intake. Fat-soluble vitamins (A, D, E, K) are also supplemented.

答え

B · With all meals and snacks that contain fat

B · Not start CPR, call for support, and provide dignity and comfort

A valid do-not-attempt-CPR decision means CPR is not attempted. Other appropriate care continues, the senior team and family are informed and the patient is cared for with dignity. Nurses should check that the decision is valid and applies to the situation.

答え

B · Not start CPR, call for support, and provide dignity and comfort

A · Thyroid function tests

Amiodarone is rich in iodine and can cause both hypothyroidism and hyperthyroidism. Thyroid function is checked before and during treatment. The symptoms here suggest hypothyroidism.

答え

A · Thyroid function tests

A · It increases bleeding, wound infection and heart complications

Even mild hypothermia impairs clotting, increases surgical site infection and cardiac events, and causes shivering and discomfort. Forced-air warming, warmed fluids and covering the patient help prevent it.

答え

A · It increases bleeding, wound infection and heart complications

A · Consider an alternative; if an NSAID is needed, add a PPI

Older age and a previous ulcer greatly increase NSAID-related bleeding. Safer options are considered first; if an NSAID is needed, the lowest effective dose for the shortest time is used with gastroprotection such as a PPI.

答え

A · Consider an alternative; if an NSAID is needed, add a PPI
