C · “I will use my steroid inhaler when I have a sudden attack.”

Inhaled corticosteroids reduce inflammation over time and do not relieve an acute attack. Salbutamol (albuterol) is the quick-relief inhaler; rinsing after steroids helps prevent oral thrush.

答え

C · “I will use my steroid inhaler when I have a sudden attack.”

D · Report the result before starting, because insulin will push potassium even lower

Insulin drives potassium into cells and can cause dangerous hypokalemia, so a low starting potassium must be reported. US (ADA) guidance delays insulin until potassium is at least 3.3 mEq/L; UK (JBDS) guidance asks for urgent senior review and potassium replacement. Either way, the prescriber decides.

答え

D · Report the result before starting, because insulin will push potassium even lower

B · The patient may climb over them or get trapped in gaps

Confused patients may climb over rails and fall from a greater height or become trapped between the rail and mattress. Alternatives such as low beds and close observation are often safer.

答え

B · The patient may climb over them or get trapped in gaps

B · Airborne and contact precautions until all lesions are crusted

Lesions over several areas on both sides mean disseminated zoster, which can spread through the air like chickenpox. Airborne and contact precautions are used until all lesions are dry and crusted. Localized zoster in an immunocompromised patient also needs airborne and contact precautions, but only until dissemination is ruled out.

答え

B · Airborne and contact precautions until all lesions are crusted

C · Just before the meal, giving insulin when the food is ready

Rapid-acting insulin starts working within about 15 minutes, so glucose is checked before the meal and the insulin is given when the meal is available, usually within 15 minutes of eating. Giving it without food can cause hypoglycemia.

答え

C · Just before the meal, giving insulin when the food is ready

B · IV thiamine

People with alcohol use disorder and malnutrition are often deficient in thiamine. Giving glucose first can use up the remaining thiamine and precipitate Wernicke encephalopathy, so thiamine is usually given before or with glucose. Hypoglycemia must still be treated promptly.

答え

B · IV thiamine

D · Constipation

Opioids slow gut movement, and constipation persists with long-term use. A regular laxative is usually prescribed together with the opioid.

答え

D · Constipation

D · Open the airway with a head tilt–chin lift or jaw thrust

In a drowsy patient the tongue can fall back and obstruct the airway, causing snoring. Simple airway manoeuvres, oxygen and, if needed, an airway adjunct are used while help is called.

答え

D · Open the airway with a head tilt–chin lift or jaw thrust

D · Check the patient and the electrodes, as this is likely artefact

Movement, loose electrodes and muscle activity can create artefact that looks like VF. A conscious, talking patient cannot be in VF. The nurse assesses the patient and checks the leads and electrodes, then reattaches them if needed.

答え

D · Check the patient and the electrodes, as this is likely artefact

A · Stop the infusion, inform the prescriber and anticipate protamine

With bleeding and a high aPTT, the heparin infusion is stopped at once and the prescriber informed. Because heparin's effect wears off within hours, stopping is often enough, but protamine sulfate can reverse it. Vitamin K reverses warfarin, not heparin.

答え

A · Stop the infusion, inform the prescriber and anticipate protamine

D · The eye surgeon must be told, as it can cause floppy iris during surgery

Tamsulosin blocks alpha-1A receptors in the iris dilator muscle as well as in the prostate, which can cause intraoperative floppy iris syndrome during cataract surgery. The effect can persist after the drug is stopped, so the surgeon must know in advance to plan the operation. Whether to pause it is a decision for the team.

答え

D · The eye surgeon must be told, as it can cause floppy iris during surgery

A · A stress ulcer of the stomach or duodenum after a major burn

Curling's ulcer is an acute peptic ulcer after severe burns, linked with reduced gut blood flow. Early enteral feeding and acid suppression help prevent it, and GI bleeding is watched for.

答え

A · A stress ulcer of the stomach or duodenum after a major burn
