A · Encourage the patient to keep coughing and stay with them

A partial obstruction with a forceful cough is best cleared by the patient’s own coughing. The nurse stays and is ready to act if the cough becomes weak or the patient cannot speak or breathe.

答え

A · Encourage the patient to keep coughing and stay with them

A · High glucose impairs white cell function and raises the risk of surgical site infection

Hyperglycemia weakens neutrophil function and slows healing, so it increases the risk of surgical site infection. Keeping glucose in the target range is part of infection prevention for surgical patients.

答え

A · High glucose impairs white cell function and raises the risk of surgical site infection

C · The source patient is tested per policy, and the nurse has baseline tests

After first aid and reporting, the source patient is tested according to law and policy, and the nurse has baseline testing. A rapid risk assessment decides whether HIV prophylaxis is started while results are awaited.

答え

C · The source patient is tested per policy, and the nurse has baseline tests

C · An aplastic crisis, often caused by parvovirus B19

Patients with sickle cell disease depend on a high reticulocyte count because their red cells live only a short time. Parvovirus B19 can stop red cell production for days, causing a rapid fall in hemoglobin with very low reticulocytes. Transfusion may be needed. Reticulocyte Count · MedlinePlus (NIH) (2024)

答え

C · An aplastic crisis, often caused by parvovirus B19

C · In standard numbered positions across the front and left of the chest

The six chest leads (V1–V6) have standard anatomical positions, from the 4th intercostal space beside the sternum to the mid-axillary line. Wrong placement can change the ECG and lead to misinterpretation.

答え

C · In standard numbered positions across the front and left of the chest

A · The cold protects the brain, so recovery may be possible after long resuscitation

Hypothermia reduces the brain's oxygen needs, and good recoveries have occurred after long resuscitation. Death is usually not confirmed until the patient has been rewarmed, unless injuries are clearly not survivable.

答え

A · The cold protects the brain, so recovery may be possible after long resuscitation

C · Vasospastic (Prinzmetal) angina

Vasospastic angina is caused by temporary coronary artery spasm, often at rest or at night, with transient ST elevation. It responds to calcium channel blockers and nitrates. Triggers include smoking, cold and cocaine.

答え

C · Vasospastic (Prinzmetal) angina

D · Respiratory alkalosis from hyperventilation

Hyperventilation blows off CO₂, causing respiratory alkalosis. The alkalosis lowers ionised calcium, causing tingling and carpopedal spasm. Normal oxygen saturation is reassuring, but organic causes such as PE, asthma or diabetic ketoacidosis must still be excluded.

答え

D · Respiratory alkalosis from hyperventilation

D · Taking pressure off the ulcer

Offloading (removing pressure) is essential for healing a neuropathic ulcer, along with infection control, good blood supply and specialist foot team care. Hot soaks risk burns because sensation is reduced.

答え

D · Taking pressure off the ulcer

A · Renal vein thrombosis

Loss of anticoagulant proteins in nephrotic syndrome raises the risk of thrombosis, including in the renal vein. This can cause loin pain, haematuria and worse kidney function. Imaging and anticoagulation are needed.

答え

A · Renal vein thrombosis

D · Eat a normal, balanced diet and introduce foods gradually

People with a colostomy can usually eat a normal diet. Foods are reintroduced gradually to see how each one affects gas, odour and output, and regular meals and fluids help.

答え

D · Eat a normal, balanced diet and introduce foods gradually

D · Potassium 2.9 mmol/L

Normal potassium is about 3.5–5.0 mmol/L. Hypokalaemia increases the risk of arrhythmias during anaesthesia and needs correction before elective surgery. The other results are within normal ranges.

答え

D · Potassium 2.9 mmol/L
