B · Patient reclined at 45° with the head turned slightly to the left
With the patient at 45°, the vertical height of the pulsation in the right internal jugular vein above the sternal angle is estimated. A height above about 3–4 cm suggests raised right-sided pressure, as in right heart failure or fluid overload.
답
B · Patient reclined at 45° with the head turned slightly to the left
D · Sternal wound infection with an unstable sternum
Deep sternal wound infection and sternal dehiscence are serious complications, more common with diabetes, obesity and COPD. Signs include fever, wound discharge, redness and an unstable or clicking sternum. Urgent surgical review, wound swabs, blood cultures and antibiotics are needed.
답
D · Sternal wound infection with an unstable sternum
A · To match the natural cortisol rhythm and protect the stomach

A · To match the natural cortisol rhythm and protect the stomach
C · Low blood volume can worsen vasospasm and brain ischaemia
Hyponatraemia after subarachnoid haemorrhage may come from SIADH or cerebral salt wasting. Restricting fluid can cause hypovolaemia, which makes delayed cerebral ischaemia more likely. Sodium is usually corrected with fluids or salt under specialist advice.
답
C · Low blood volume can worsen vasospasm and brain ischaemia
A · They can reduce the absorption of many other drugs

A · They can reduce the absorption of many other drugs
B · Pulmonary embolism; sit up, give oxygen and call for urgent help
Sudden pleuritic chest pain, breathlessness, tachycardia and hypoxia after surgery strongly suggest PE. The nurse supports breathing with oxygen, escalates urgently and prepares for investigations and anticoagulation.
답
B · Pulmonary embolism; sit up, give oxygen and call for urgent help
답: D · Just before or with a meal
Rapid-acting insulins start working within minutes, so they are given with meals to match the glucose rise. Giving the dose without eating risks hypoglycaemia.
답
D · Just before or with a meal

Sedatives, hypnotics, antipsychotics and drugs that lower blood pressure increase falls risk. A medication review is part of every falls assessment.
답
A · Benzodiazepine sleeping tablets
A · Heart failure is an unlikely cause of the breathlessness
BNP is released from stretched ventricles, so a low value makes heart failure an unlikely cause of acute breathlessness. A high value supports heart failure, but BNP also rises with age, kidney disease and pulmonary embolism. Natriuretic Peptide Tests (BNP, NT-proBNP) · MedlinePlus (NIH) (2023)
답
A · Heart failure is an unlikely cause of the breathlessness
Low blood sugar · MedlinePlus (NIH) (2026)
HCO₃⁻ = 21.4 mmol/L
|27 − 40| = 13 mmHg → 1.3 × 10 mmHg
예상 HCO₃⁻
HCO₃⁻ = 24 − 2·1.3 = 21.4 mmol/L
답
HCO₃⁻ = 21.4 mmol/L
답: A–a 기울기 = 164.9 mmHg
해수면에서 폐포 산소 분압은 PAO₂ = FiO₂·(760 − 47) − PaCO₂/0.8 mmHg이고, A–a 기울기는 PAO₂ − PaO₂입니다.
QT = 9.5 × 0.04 = 0.38 s
RR = 17 × 0.04 = 0.68 s
Fridericia 공식
QTc = 0.38/∛0.68 = 0.38/0.8794 = 0.432 s
답
QTc = 0.432 s
B · Heparin-induced thrombocytopenia
Heparin-induced thrombocytopenia usually appears 5–10 days after starting heparin, with a large fall in platelets and a risk of clotting. The nurse reports at once so heparin can be stopped.
답
B · Heparin-induced thrombocytopenia
C · Dantrolene
Malignant hyperthermia is a reaction to some anesthetic agents that causes hypermetabolism. The triggering drug is stopped and dantrolene is given, with active cooling and support.
