D · Hold the next dose and report for an urgent lithium level
Vomiting, diarrhea and a coarse tremor suggest lithium toxicity. The nurse holds the dose and reports so a serum level can be checked; low salt or fluid intake raises lithium levels further.
D · Hold the next dose and report for an urgent lithium level
D · Hemoglobin 6.5 g/dL (65 g/L) in a patient with a GI bleed
A hemoglobin of 6.5 g/dL (65 g/L) in a bleeding patient shows severe anemia and may need urgent transfusion. The other results are mildly abnormal or stable.
D · Hemoglobin 6.5 g/dL (65 g/L) in a patient with a GI bleed
C · 0.9% sodium chloride
Only 0.9% sodium chloride is compatible with blood. Dextrose (glucose) solutions can cause red cells to clump and break down, and the calcium in lactated Ringer’s (Hartmann’s) can cause clotting.
A · Decontaminate them outside, with staff wearing appropriate PPE
Contaminated patients are decontaminated outside the department so the chemical is not brought inside to harm staff and other patients. Staff doing this wear PPE suited to the hazard.
A · Decontaminate them outside, with staff wearing appropriate PPE
D · Crackles, hypertension and a raised BNP
TACO is fluid overload, often in older adults or those with heart or kidney disease. It causes dyspnea, crackles, hypertension, distended neck veins and a raised BNP. The transfusion is stopped, the patient is sat up and diuretics may be prescribed.
D · Crackles, hypertension and a raised BNP
B · Hypokalemia from gastric losses is contributing to paralytic ileus
Gastric suction removes potassium and causes metabolic alkalosis, which shifts more potassium into cells. Low potassium reduces smooth muscle contraction and contributes to ileus and weakness. The provider is informed and potassium is replaced as prescribed. Hypokalemia · StatPearls (NCBI Bookshelf) (2025)
B · Hypokalemia from gastric losses is contributing to paralytic ileus
답: A · Low sodium and high potassium
Without aldosterone, the kidneys lose sodium and retain potassium. Patients may also have low blood pressure, low glucose and increased skin pigmentation.
A · Low sodium and high potassium
답: D · "I will rub the area firmly after the injection."
Rubbing the site after an anticoagulant injection increases bruising and is avoided. Rotating sites, injecting subcutaneously and reporting bleeding are correct.
D · "I will rub the area firmly after the injection."
C · Shout for help, open the airway and check breathing for up to 10 seconds
In hospital, the nurse shouts for help, opens the airway and looks for normal breathing (and a pulse if trained) for up to 10 seconds. If the patient is not breathing normally, the arrest team is called and CPR started, with a defibrillator brought as soon as possible.
C · Shout for help, open the airway and check breathing for up to 10 seconds
C · To improve the strength of chewing and swallowing at mealtimes
Pyridostigmine inhibits acetylcholinesterase, so more acetylcholine is available at the neuromuscular junction. Timing it before meals gives the best muscle strength for chewing and swallowing, which lowers the risk of aspiration.
C · To improve the strength of chewing and swallowing at mealtimes
D · Raised pressure in the skull, so the procedure could cause herniation
Reduced consciousness and unequal pupils suggest raised ICP. A lumbar puncture in this situation may cause brain herniation, so imaging comes first, and antibiotics are not delayed. The nurse should raise this concern.
D · Raised pressure in the skull, so the procedure could cause herniation
B · Normal protein and enough calories, with a late-evening snack
Protein restriction worsens malnutrition and muscle loss and is no longer routinely advised. Adequate protein and energy, small frequent meals and a late-evening snack are recommended.
B · Normal protein and enough calories, with a late-evening snack
A · Stop the preparation and escalate to the medical team
Bowel preparation can cause dehydration and electrolyte disturbance, including hyponatraemia, especially in older people. Confusion with low sodium needs prompt medical review, and further prep is withheld until assessed.
A · Stop the preparation and escalate to the medical team
