Infekcijų kontrolė ir pacientų sauga

Metodas

  1. Pradėkite nuo standartinių atsargumo priemonių kiekvienam pacientui: rankų higiena, asmeninės apsaugos priemonės numatomai ekspozicijai, saugūs aštrūs daiktai.
  2. Pridėkite atsargumo priemones pagal plitimo kelią: kontaktinė (chalatas, pirštinės), lašelinė (chirurginė kaukė), oro (N95, neigiamo slėgio kambarys).
  3. Steriliems daiktams išlaikykite sterilumą: jei abejojate, jis nesterilus; pakeiskite, niekada nenuvalykite.

2Uždaviniai

1.Infekcijų kontrolė ir pacientų saugaVidutinis

A patient has fallen, and the nurse completes an incident report. Which statement about this report is correct?

  1. AA copy of it is placed in the patient’s chart so all staff know about the fall
  2. BIt replaces the need to document the fall in the patient’s record
  3. CIt is only needed when the patient is injured
  4. DIt is written factually and does not replace documenting the fall in the patient’s record
2.Infekcijų kontrolė ir pacientų saugaSunkus

According to the “Swiss cheese” model of error, why do errors reach patients?

  1. AOne careless person makes a mistake that no system could stop
  2. BWeaknesses in several layers of defense line up at the same time
  3. CPatients often do not follow their care instructions
  4. DTechnology fails more often than people do
3.Slaugos pagrindaiVidutinis

How should a nurse speak to a patient with hearing loss?

  1. AFace the patient and speak clearly at a normal pace
  2. BSpeak very fast to keep the message short
  3. CShout loudly from behind the patient so they can hear every word
  4. DCover the mouth to stop background noise
4.Slaugos pagrindaiSunkus

For a patient at risk of pressure ulcers, why is the head of the bed usually kept at 30 degrees or less if their condition allows?

  1. ALower angles increase blood flow to the heels
  2. BLower angles stop the patient from getting out of bed
  3. CHigher angles always cause breathing problems in patients who are bedbound
  4. DHigher angles make the patient slide down, causing shear over the sacrum
5.MikrobiologijaVidutinis

Kas yra inkubacinis laikotarpis?

  1. ALaikas, kurio reikia pasėliui laboratorijoje
  2. BLaikas, per kurį pradeda veikti antibiotikas
  3. CLaikas, kurį pacientas lieka užkrečiamas kitiems
  4. DLaikas nuo užsikrėtimo iki pirmųjų simptomų
6.MikrobiologijaSunkus

1860-aisiais Josephas Listeris sumažino infekcijų po operacijų skaičių. Kokia buvo pagrindinė jo naujovė?

  1. AKarbolio rūgšties (fenolio) naudojimas kaip antiseptiko žaizdoms ir instrumentams
  2. BVisų chirurginių instrumentų sterilizavimas garo autoklavais prieš kiekvieną operaciją
  3. CChirurgų skiepijimas nuo stafilokokų
  4. DSulfonamidų antibiotikų skyrimas po kiekvienos operacijos

1.Infekcijų kontrolė ir pacientų sauga

Atsakymas: D · It is written factually and does not replace documenting the fall in the patient’s record

Kodėl tai yra atsakymas

The nurse documents the facts of the fall, the assessment and the care given in the patient’s record. The incident report is a separate quality-improvement document and is not filed in the chart. Policies differ on whether the record notes that a report was made: many US facilities advise against it, while many UK services record the report reference. Falls are reported whether or not there is an injury.

Atsakymas

D · It is written factually and does not replace documenting the fall in the patient’s record
2.Infekcijų kontrolė ir pacientų sauga

Atsakymas: B · Weaknesses in several layers of defense line up at the same time

Kodėl tai yra atsakymas

Reason’s Swiss cheese model shows that each safety layer, such as prescribing, dispensing, checking and scanning, has weaknesses. Harm happens when the holes in several layers line up, so safety improvement focuses on strengthening the system.

Atsakymas

B · Weaknesses in several layers of defense line up at the same time
3.Slaugos pagrindai

Atsakymas: A · Face the patient and speak clearly at a normal pace

Kodėl tai yra atsakymas

Facing the patient in good light, reducing background noise and speaking clearly, without shouting, helps understanding. Hearing aids should be in and working.

Atsakymas

A · Face the patient and speak clearly at a normal pace
4.Slaugos pagrindai

Atsakymas: D · Higher angles make the patient slide down, causing shear over the sacrum

Kodėl tai yra atsakymas

When the head is raised high, the body tends to slide while the skin stays still, distorting the tissues over the sacrum. Keeping the angle low, where safe, reduces this shear.

Atsakymas

D · Higher angles make the patient slide down, causing shear over the sacrum
5.Mikrobiologija

Atsakymas: D · Laikas nuo užsikrėtimo iki pirmųjų simptomų

Kodėl tai yra atsakymas

Inkubacinis laikotarpis trunka nuo sąlyčio iki simptomų pradžios. Kartais infekcija gali būti perduodama jau šiuo laikotarpiu.

Atsakymas

D · Laikas nuo užsikrėtimo iki pirmųjų simptomų
6.Mikrobiologija

Atsakymas: A · Karbolio rūgšties (fenolio) naudojimas kaip antiseptiko žaizdoms ir instrumentams

Kodėl tai yra atsakymas

Listeris karbolio rūgštimi apdorodavo žaizdas, tvarsčius ir instrumentus, todėl pooperacinių infekcijų labai sumažėjo. Taip prasidėjo antiseptinė chirurgija.

Atsakymas

A · Karbolio rūgšties (fenolio) naudojimas kaip antiseptiko žaizdoms ir instrumentams
