1Formula dan kaedah

Pemberian keutamaan & delegasi

Kaedah

Kawalan jangkitan & keselamatan pesakit

Kaedah

  2. Tambah langkah berjaga-jaga mengikut laluan: sentuhan (gaun, sarung tangan), titisan (pelitup muka pembedahan), bawaan udara (N95, bilik tekanan negatif).
  3. Pastikan item steril kekal steril: jika ragu-ragu, ia tidak steril; gantikan, jangan sekali-kali dilap.

Perubatan-pembedahan: neurologi, endokrin & renal

Kaedah

1.Pemberian keutamaan & delegasi

Jawapan: A · Do not measure blood pressure on the left arm

Mengapa ini jawapannya

Blood pressure cuffs, tight bands and heavy loads can damage or block a fistula. The arm with the access is protected, and the nurse checks it for a thrill and bruit.

Jawapan

A · Do not measure blood pressure on the left arm
2.Pemberian keutamaan & delegasi

Jawapan: B · Serotonin syndrome

Mengapa ini jawapannya

SSRIs and tramadol both raise serotonin. Agitation, fever, clonus and hyperreflexia that start quickly suggest serotonin syndrome; neuroleptic malignant syndrome follows antipsychotics and causes rigid muscles with slow reflexes.

Jawapan

B · Serotonin syndrome
3.Kawalan jangkitan & keselamatan pesakit

Jawapan: C · Whether the patient has any metal implants or devices, such as a pacemaker

Mengapa ini jawapannya

The MRI magnet can move or heat metal objects and can stop some implanted devices from working. Patients are screened for implants, pacemakers, metal fragments and metal items such as jewelry before entering the scanner room.

Jawapan

C · Whether the patient has any metal implants or devices, such as a pacemaker
4.Kawalan jangkitan & keselamatan pesakit

Jawapan: B · Stop the infusion and check the patient’s blood glucose

Mengapa ini jawapannya

A concentration error means the dose delivered was wrong. The nurse stops the infusion, assesses the patient including blood glucose, treats as needed, notifies the prescriber and reports the event.

Jawapan

B · Stop the infusion and check the patient’s blood glucose
5.Nilai makmal & elektrolit

Jawapan: A · About 30 minutes after the infusion ends

Mengapa ini jawapannya

Peak aminoglycoside levels are usually drawn about 30 minutes after a 30-minute infusion ends, to confirm the level is high enough to kill bacteria. Troughs, drawn just before the next dose, check for accumulation. Local protocols may differ, especially with once-daily dosing.

Jawapan

A · About 30 minutes after the infusion ends
6.Nilai makmal & elektrolit

Jawapan: D · Pendarahan

Jawapan

D · Pendarahan
7.Kejururawatan klinikal

Jawapan: C · It lowers the oncotic pressure that holds fluid in the blood vessels

Mengapa ini jawapannya

Albumin creates oncotic pressure, drawing fluid into the capillaries. When it is low, as in liver disease, nephrotic syndrome or malnutrition, fluid leaks into the tissues.

Jawapan

C · It lowers the oncotic pressure that holds fluid in the blood vessels
8.Kejururawatan klinikal

Jawapan: C · Untuk mengurangkan risiko disfungsi peredaran selepas pengeluaran cecair isi padu besar

Infusi albumin membantu mengekalkan isi padu peredaran.

Jawapan

C · Untuk mengurangkan risiko disfungsi peredaran selepas pengeluaran cecair isi padu besar
9.Perubatan-pembedahan: kardiovaskular & respiratori

Jawapan: A · Take the first dose at bedtime, as dizziness and fainting can occur

Mengapa ini jawapannya

Alpha-blockers can cause marked first-dose postural hypotension. Taking the first dose at bedtime and rising slowly reduces the risk of falls. They are also used for symptoms of an enlarged prostate.

Jawapan

A · Take the first dose at bedtime, as dizziness and fainting can occur
10.Perubatan-pembedahan: kardiovaskular & respiratori

Jawapan: C · Stop or clamp the drainage and inform the doctor

Mengapa ini jawapannya

Rapid removal of a large effusion can cause re-expansion pulmonary oedema. Coughing, chest tightness or pain are warning signs. Drainage is stopped and the doctor is informed, and the volume is usually drained in a controlled way.

Jawapan

C · Stop or clamp the drainage and inform the doctor
11.Perubatan-pembedahan: neurologi, endokrin & renal

Jawapan: C · It takes months to work; pregnant women must not handle broken tablets

Mengapa ini jawapannya

Finasteride, a 5-alpha reductase inhibitor, reduces the conversion of testosterone to dihydrotestosterone and shrinks the prostate over several months. It can harm a male fetus, so pregnant women should avoid handling crushed or broken tablets.

Jawapan

C · It takes months to work; pregnant women must not handle broken tablets
12.Perubatan-pembedahan: neurologi, endokrin & renal

Jawapan: D · A patient with a potassium of 6.8 mmol/L who reports palpitations

Mengapa ini jawapannya

Severe hyperkalaemia with palpitations may lead to life-threatening arrhythmias and needs an urgent ECG, cardiac monitoring and treatment. The other problems are stable or less urgent.

Jawapan

D · A patient with a potassium of 6.8 mmol/L who reports palpitations
13.Perubatan-pembedahan: GI, perioperatif & onkologi

Jawapan: C · Return of leg sensation and movement, and passing urine

Mengapa ini jawapannya

The nurse monitors the level of the block, recovery of leg movement and sensation, BP and bladder function. Urinary retention is common, and early mobilisation is only safe once motor function returns.

Jawapan

C · Return of leg sensation and movement, and passing urine
14.Perubatan-pembedahan: GI, perioperatif & onkologi

Jawapan: D · Potassium 6.0 mmol/L

Mengapa ini jawapannya

Spironolactone blocks aldosterone and can cause hyperkalaemia, which risks arrhythmias. A potassium of 6.0 mmol/L is above the normal range (about 3.5–5.0) and needs urgent review before further doses.

Jawapan

D · Potassium 6.0 mmol/L
