1Formula dan kaedah

Pemberian keutamaan & delegasi

Kaedah

Kawalan jangkitan & keselamatan pesakit

Kaedah

  2. Tambah langkah berjaga-jaga mengikut laluan: sentuhan (gaun, sarung tangan), titisan (pelitup muka pembedahan), bawaan udara (N95, bilik tekanan negatif).
  3. Pastikan item steril kekal steril: jika ragu-ragu, ia tidak steril; gantikan, jangan sekali-kali dilap.

Perubatan-pembedahan: neurologi, endokrin & renal

Kaedah

1.Pemberian keutamaan & delegasi

Jawapan: A · A structured format, such as SBAR, with time for questions

Mengapa ini jawapannya

Handover is a high-risk moment for errors. A structured format and the chance to ask questions help the next nurse understand each patient’s condition and priorities.

Jawapan

A · A structured format, such as SBAR, with time for questions
2.Pemberian keutamaan & delegasi

Jawapan: C · Loosen the restraint and assess the circulation

Mengapa ini jawapannya

Pale, cool fingers with tingling suggest that the restraint is too tight and blood flow is reduced. The nurse releases it at once, assesses the hand and reviews whether the restraint is still needed.

Jawapan

C · Loosen the restraint and assess the circulation
3.Kawalan jangkitan & keselamatan pesakit

Jawapan: B · The patient may climb over them or get trapped in gaps

Mengapa ini jawapannya

Confused patients may climb over rails and fall from a greater height or become trapped between the rail and mattress. Alternatives such as low beds and close observation are often safer.

Jawapan

B · The patient may climb over them or get trapped in gaps
4.Kawalan jangkitan & keselamatan pesakit

Jawapan: D · State the concern clearly a second time and, if still unresolved, use the chain of command

Mengapa ini jawapannya

The two-challenge rule says the concern is voiced at least twice to make sure it was heard. If it is still not addressed, the nurse escalates through the chain of command rather than acting against the patient’s safety.

Jawapan

D · State the concern clearly a second time and, if still unresolved, use the chain of command
5.Nilai makmal & elektrolit

Jawapan: B · Send the fluid for cell count and culture and report it

Mengapa ini jawapannya

Cloudy effluent with abdominal pain suggests peritonitis, the main serious complication of peritoneal dialysis. A sample is sent for white cell count, Gram stain and culture, and antibiotics (often given in the dialysis fluid) are started as prescribed.Disemak dengan: Dialysis - peritoneal · MedlinePlus (NIH) (2024)

Jawapan

B · Send the fluid for cell count and culture and report it
6.Nilai makmal & elektrolit

Jawapan: B · "I can stop my blood tests since I feel well"

Mengapa ini jawapannya

Clozapine requires ongoing ANC monitoring according to the schedule, because severe neutropenia can occur without warning. The patient must keep blood test appointments and report signs of infection at once.

Jawapan

B · "I can stop my blood tests since I feel well"
7.Kejururawatan klinikal

Jawapan: D · That the cannula is intact, and apply pressure until bleeding stops

Mengapa ini jawapannya

The nurse checks that the whole cannula has been removed, applies pressure and a dressing, and records the removal. Any signs of phlebitis are noted.

Jawapan

D · That the cannula is intact, and apply pressure until bleeding stops
8.Kejururawatan klinikal

Jawapan: B · It stops the clot growing and prevents new clots while the body breaks it down

Mengapa ini jawapannya

Anticoagulants prevent clot extension and new clots; the body's own fibrinolysis gradually breaks down the existing clot. Thrombolysis, which actively dissolves clots, is reserved for massive PE with shock.

Jawapan

B · It stops the clot growing and prevents new clots while the body breaks it down
9.Perubatan-pembedahan: kardiovaskular & respiratori

10.Perubatan-pembedahan: kardiovaskular & respiratori

Jawapan: C · A sustained waveform on capnography

Mengapa ini jawapannya

A sustained exhaled carbon dioxide waveform confirms tracheal placement. Misting and chest movement can be misleading, and skin colour changes late. A chest X-ray shows the depth of the tube but does not prove it is in the trachea.

Jawapan

C · A sustained waveform on capnography
11.Perubatan-pembedahan: neurologi, endokrin & renal

Jawapan: D · To match the natural cortisol rhythm and protect the stomach

Mengapa ini jawapannya

Morning dosing mimics the normal early-morning cortisol peak, causes less adrenal suppression and less sleep disturbance. Taking it with food reduces gastric irritation.

Jawapan

D · To match the natural cortisol rhythm and protect the stomach
12.Perubatan-pembedahan: neurologi, endokrin & renal

Jawapan: B · Loss of the outer half of the field in both eyes

Mengapa ini jawapannya

The chiasm carries crossing fibres from the nasal retinas, which see the temporal (outer) fields. Compression causes a bitemporal hemianopia. Patients may bump into objects at their sides.

Jawapan

B · Loss of the outer half of the field in both eyes
13.Perubatan-pembedahan: GI, perioperatif & onkologi

Jawapan: C · So that carriers can be decolonised to reduce infection

Mengapa ini jawapannya

MRSA carriers have a higher risk of surgical site infection. Screening allows decolonisation with nasal ointment and skin washes, and the right antibiotic prophylaxis.

Jawapan

C · So that carriers can be decolonised to reduce infection
14.Perubatan-pembedahan: GI, perioperatif & onkologi

Jawapan: D · Withhold the next dose and report to the prescriber

Mengapa ini jawapannya

NSAIDs can cause acute kidney injury, particularly with hypovolaemia, older age or other nephrotoxic drugs. The nurse withholds and reports, and fluid status and other analgesia are reviewed.

Jawapan

D · Withhold the next dose and report to the prescriber
