Infectiepreventie en patiëntveiligheid

Methode

Before a blood transfusion is started, which check is required?

  1. AThe patient signs the blood bag label to confirm their name before it is hung
  2. BThe nurse checks only that the bag number matches the order
  3. CThe patient’s identity is checked against the blood product and its paperwork at the bedside
  4. DOne nurse checks the blood product against the order at the nurses’ station

A patient receiving chemotherapy has an absolute neutrophil count of 400/mm3 (0,4 × 109/L) and a new temperature of 38,3 °C (100,9 °F). What should the nurse do?

  1. AEncourage oral fluids and record the result in the chart
  2. BRecheck the temperature in four hours before calling anyone about it
  3. CReport it at once, because fever with severe neutropenia is an emergency
  4. DGive an antipyretic and reassess at the next routine round
3.BasisverpleegkundeGemiddeld

What is the correct order of techniques when assessing the abdomen?

  1. APalpation, inspection, auscultation, percussion
  2. BAuscultation, palpation, inspection, percussion
  3. CInspection, palpation, percussion, auscultation
  4. DInspection, auscultation, percussion, palpation
4.BasisverpleegkundeMoeilijk

5.MicrobiologieGemiddeld

Welke bacterie veroorzaakt kinkhoest (pertussis)?

  1. ABordetella pertussis
  2. BHaemophilus influenzae
  3. CCorynebacterium diphtheriae
  4. DMycoplasma pneumoniae
6.MicrobiologieMoeilijk

Primaire syfilis presenteert zich meestal met:

  2. BEen uitgebreide huiduitslag op handpalmen en voetzolen
  4. DSchade aan hart en hersenen

3Oplossingen

1.Infectiepreventie en patiëntveiligheid

Antwoord: C · The patient’s identity is checked against the blood product and its paperwork at the bedside

Waarom dit het antwoord is

Wrong-patient transfusion can cause a fatal ABO reaction, so the final check is always done at the bedside, with positive patient identification against the product label, compatibility tag and order. Many US hospitals require two qualified people for this check, while UK guidance allows one trained person if positive identification is used. Local policy decides who checks.

Antwoord

C · The patient’s identity is checked against the blood product and its paperwork at the bedside
2.Infectiepreventie en patiëntveiligheid

Antwoord: C · Report it at once, because fever with severe neutropenia is an emergency

Waarom dit het antwoord is

Febrile neutropenia can progress to sepsis quickly. The nurse reports it immediately so cultures can be taken and antibiotics started without delay.

Antwoord

C · Report it at once, because fever with severe neutropenia is an emergency
3.Basisverpleegkunde

Antwoord: D · Inspection, auscultation, percussion, palpation

Waarom dit het antwoord is

In the abdomen the nurse listens before touching, because percussion and palpation can change bowel sounds. In other body systems palpation and percussion come before auscultation.

Antwoord

D · Inspection, auscultation, percussion, palpation
4.Basisverpleegkunde

5.Microbiologie

Antwoord: A · Bordetella pertussis

Antwoord

A · Bordetella pertussis
6.Microbiologie
