An order uses the abbreviation “MS”. Why is this unsafe?
AIt is the abbreviation for milliseconds, not a drug
BIt can mean either morphine sulfate or magnesium sulfate
CIt means the drug must be given by mouth only
DIt is reserved for drugs used in multiple sclerosis
2.Infectiepreventie en patiëntveiligheidMoeilijk
An 84-year-old resident with an indwelling catheter suddenly becomes confused but has no fever. What should the nurse consider?
AInfection can be ruled out because there is no fever
BAn infection may present as confusion in older adults, so assess further and report
CA sedative is needed to keep the resident calm
DThe confusion is part of normal aging at this age and needs no further action
3.BasisverpleegkundeGemiddeld
Why are patients with a new ileostomy at risk of dehydration?
AThe stoma absorbs water from the body
BThe colon removes too much water from the stool
CThey are not allowed to drink for several weeks after surgery
DThe output is liquid and contains a lot of water and salt
4.BasisverpleegkundeMoeilijk
A patient with type 1 diabetes is vomiting and not eating. Which advice is correct?
AStop all insulin until eating normally again
BKeep taking basal insulin and check glucose and ketones more often
CHalve every dose and stop testing the glucose
DDrink only sugar-free fluids and stop testing for ketones until better
5.MicrobiologieGemiddeld
BEnzymatische inactivatie
CEffluxpompen
DAlleen verminderde opname via porines
3Oplossingen
1.Infectiepreventie en patiëntveiligheid
Antwoord: B · It can mean either morphine sulfate or magnesium sulfate
Waarom dit het antwoord is
“MS”, “MSO4” and “MgSO4” can be confused, and giving one drug instead of the other can cause serious harm. Drug names are written in full.
Antwoord
B · It can mean either morphine sulfate or magnesium sulfate
2.Infectiepreventie en patiëntveiligheid
Antwoord: B · An infection may present as confusion in older adults, so assess further and report
Waarom dit het antwoord is
In older adults, infections such as urinary tract infection may present with new confusion, falls or reduced appetite rather than fever. Sudden confusion is assessed and reported, not dismissed as aging.
Antwoord
B · An infection may present as confusion in older adults, so assess further and report
3.Basisverpleegkunde
Antwoord: D · The output is liquid and contains a lot of water and salt
Waarom dit het antwoord is
The colon normally reabsorbs water and salt. An ileostomy bypasses it, so high output can quickly cause dehydration and low sodium; output is monitored and fluids encouraged.
Antwoord
D · The output is liquid and contains a lot of water and salt
4.Basisverpleegkunde
Antwoord: B · Keep taking basal insulin and check glucose and ketones more often
Waarom dit het antwoord is
Sick-day rules say people with type 1 diabetes should never stop their insulin, as illness increases glucose and the risk of ketoacidosis. Frequent testing and early advice are needed.
Antwoord
B · Keep taking basal insulin and check glucose and ketones more often
5.Microbiologie
