Infectiepreventie en patiëntveiligheid

Methode

2Oefenopgaven

1.Infectiepreventie en patiëntveiligheidGemiddeld

A patient with delirium is at risk of falls and of pulling out lines. Which option is less restrictive than restraints?

  1. AA trained sitter or video monitoring with staff nearby
  2. BA vest restraint applied for the whole night
  3. CFour side rails raised and the door closed so the patient stays in bed
  4. DA sedative given every few hours as needed
2.Infectiepreventie en patiëntveiligheidMoeilijk

A nurse calculates a child’s prescribed dose and finds it is ten times higher than the usual dose for the child’s weight. What should the nurse do?

  1. AGive it, because the prescriber must have a reason
  2. BHold the dose and verify it with the prescriber and pharmacist
  3. CGive half the dose and watch the child closely
  4. DGive one-tenth of the dose without telling anyone about it
3.BasisverpleegkundeGemiddeld

A patient tells the nurse about abuse and asks them to keep it a secret. What should the nurse say?

  1. AThat they will tell everyone on the ward so the patient is safe
  2. BThat they promise to keep it secret, so the patient keeps trusting them
  3. CThat they cannot listen to this and the patient should tell a doctor
  4. DThat they cannot promise secrecy if someone is at risk of harm
4.BasisverpleegkundeMoeilijk

A patient has a temperature of 38,7 °C and a white cell count of 18 × 10⁹/L (18,000/mm³). What do these findings most suggest?

  1. AA low risk of infection because the count is high
  2. BA normal post-meal result
  3. CDehydration only
  4. DAn infection or significant inflammation
5.MicrobiologieGemiddeld

Vaccins tegen tetanus en difterie zijn toxoïden. Wat betekent dat?

  3. CHij leeft alleen in rode bloedcellen

3Oplossingen

1.Infectiepreventie en patiëntveiligheid

Antwoord: A · A trained sitter or video monitoring with staff nearby

Waarom dit het antwoord is

Close observation by a sitter or video monitoring allows staff to step in quickly, without restricting the patient. It is often combined with reorientation and family presence.

Antwoord

A · A trained sitter or video monitoring with staff nearby
2.Infectiepreventie en patiëntveiligheid

Antwoord: B · Hold the dose and verify it with the prescriber and pharmacist

Waarom dit het antwoord is

Tenfold errors are common in pediatrics, usually from decimal or unit mistakes. The nurse holds the dose and clarifies it, and never changes the dose on their own.

Antwoord

B · Hold the dose and verify it with the prescriber and pharmacist
3.Basisverpleegkunde

Antwoord: D · That they cannot promise secrecy if someone is at risk of harm

Waarom dit het antwoord is

Nurses must not promise secrecy when there is a risk of harm. They listen, stay calm, explain what will happen, record the disclosure and follow safeguarding procedures.

Antwoord

D · That they cannot promise secrecy if someone is at risk of harm
4.Basisverpleegkunde

Antwoord: D · An infection or significant inflammation

Waarom dit het antwoord is

A raised white cell count with fever usually means the body is fighting an infection or major inflammation. The nurse reports it and looks for the source, including sepsis.

Antwoord

D · An infection or significant inflammation
5.Microbiologie
