2Oefenopgaven
1.Infectiepreventie en patiëntveiligheidGemiddeld
A patient has fallen, and the nurse completes an incident report. Which statement about this report is correct?
AIt is only needed when the patient is injured
BIt replaces the need to document the fall in the patient’s record
CIt is written factually and does not replace documenting the fall in the patient’s record
DA copy of it is placed in the patient’s chart so all staff know about the fall
2.Infectiepreventie en patiëntveiligheidMoeilijk
During bedside handoff, the patient says, “I’m allergic to sulfa drugs,” but no allergy is recorded. What should the nurse do?
ATell the patient that sulfa drugs are always safe
BClarify the reaction, update the allergy record and tell the prescriber
CWait until the patient is discharged to update the record
DIgnore it, because the electronic record is more reliable than the patient’s memory
3.BasisverpleegkundeGemiddeld
When measuring a wound, how is the length usually taken?
AAround the edge of the wound with a tape measure
BIn whichever direction is shortest
CFrom the wound to the nearest joint
DIn the head-to-toe direction across the longest part
4.BasisverpleegkundeMoeilijk
A collapsed patient is unresponsive and making only occasional slow gasps. What should the nurse do?
AGive a sip of water to clear the throat
BTreat it as not breathing normally, call for help and start CPR
CPlace the patient in the recovery position and keep observing them
DWait to see whether the gasps become regular
5.MicrobiologieGemiddeld
AClostridioides difficile-infectie
BSalmonella-infectie
CHelicobacter pylori-infectie
DHepatitis A
6.MicrobiologieMoeilijk
ANeisseria gonorrhoeae
BBordetella pertussis
CStreptococcus pneumoniae
DClostridium perfringens
3Oplossingen
1.Infectiepreventie en patiëntveiligheid
Antwoord: C · It is written factually and does not replace documenting the fall in the patient’s record
Waarom dit het antwoord is
The nurse documents the facts of the fall, the assessment and the care given in the patient’s record. The incident report is a separate quality-improvement document and is not filed in the chart. Policies differ on whether the record notes that a report was made: many US facilities advise against it, while many UK services record the report reference. Falls are reported whether or not there is an injury.
Antwoord
C · It is written factually and does not replace documenting the fall in the patient’s record
2.Infectiepreventie en patiëntveiligheid
Antwoord: B · Clarify the reaction, update the allergy record and tell the prescriber
Waarom dit het antwoord is
Bedside handoff lets patients correct errors. The nurse asks about the reaction, updates the record and alerts the prescriber and pharmacy so current and future orders can be checked.
Antwoord
B · Clarify the reaction, update the allergy record and tell the prescriber
3.Basisverpleegkunde
Antwoord: D · In the head-to-toe direction across the longest part
Waarom dit het antwoord is
Using the body's head-to-toe axis for length and side-to-side for width keeps measurements consistent between nurses, so changes in size are meaningful.
Antwoord
D · In the head-to-toe direction across the longest part
4.Basisverpleegkunde
Antwoord: B · Treat it as not breathing normally, call for help and start CPR
Waarom dit het antwoord is
Agonal gasps are common in the first minutes after cardiac arrest. They must not be mistaken for breathing; CPR is started and an AED fetched straight away.
Antwoord
B · Treat it as not breathing normally, call for help and start CPR
5.Microbiologie
Antwoord: A · Clostridioides difficile-infectie
Antwoord
A · Clostridioides difficile-infectie
6.Microbiologie
Antwoord: D · Clostridium perfringens
