1Formules en methoden

Prioriteren en delegeren

Methode

Antwoord: C · The confused patient climbing out of bed

Waarom dit het antwoord is

A confused patient climbing out of bed could fall and be badly injured within moments. Pain needs prompt treatment, but the immediate safety risk comes first.

Antwoord

C · The confused patient climbing out of bed
3.Infectiepreventie en patiëntveiligheid

Antwoord: B · Contact the local poison advice service now for specific advice

Waarom dit het antwoord is

A poison advice service (poison control in the US, or NHS 111 in the UK) can give advice for the exact product. Inducing vomiting is not recommended, because corrosive products can burn the esophagus again and vomit can be aspirated. Emergency services are called if the child has trouble breathing or is drowsy.

Antwoord

B · Contact the local poison advice service now for specific advice
4.Infectiepreventie en patiëntveiligheid

Antwoord: D · Explain that fatigue would make care unsafe and decline the extra shift

Waarom dit het antwoord is

Long hours and fatigue increase errors and injuries. Nurses have a professional responsibility to judge whether they are fit to work safely and to say so clearly.

Antwoord

D · Explain that fatigue would make care unsafe and decline the extra shift
5.Laboratoriumwaarden en elektrolyten

Antwoord: D · Put seizure precautions in place and report the result

Waarom dit het antwoord is

Hypocalcemia increases neuromuscular excitability and can cause tetany, laryngospasm and seizures. The nurse reports the result, keeps the patient safe with seizure precautions and expects calcium replacement. The other measures fit hypercalcemia.

Antwoord

D · Put seizure precautions in place and report the result
6.Laboratoriumwaarden en elektrolyten

Antwoord: B · Low hemoglobin, high reticulocytes, high unconjugated bilirubin, high LDH

Waarom dit het antwoord is

When red cells break down early, hemoglobin is turned into unconjugated bilirubin and LDH is released, while the marrow increases reticulocytes. Option B fits iron deficiency, C fits B12 deficiency and D fits marrow failure.

Antwoord

B · Low hemoglobin, high reticulocytes, high unconjugated bilirubin, high LDH
7.Klinische verpleegkunde

Antwoord: A · Venturi mask

Waarom dit het antwoord is

A Venturi mask entrains a set amount of room air, giving a fixed oxygen percentage regardless of the patient's breathing pattern. This makes it useful for patients at risk of hypercapnia.

Antwoord

A · Venturi mask
8.Klinische verpleegkunde

Antwoord: B · "If I forget a dose, I'll take two the next day to catch up."

Waarom dit het antwoord is

Doubling up on an anticoagulant increases the bleeding risk. Patients follow the specific missed-dose advice for their drug and never take a double dose.

Antwoord

B · "If I forget a dose, I'll take two the next day to catch up."
9.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: A · Phaeochromocytoma

Waarom dit het antwoord is

Phaeochromocytoma is a catecholamine-secreting tumour, usually of the adrenal medulla. Classic features are paroxysmal headache, sweating, palpitations and hypertension. It is diagnosed with plasma or urine metanephrines.

Antwoord

A · Phaeochromocytoma
10.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: A · Urgent escalation for CT of the head and anticoagulant reversal

Waarom dit het antwoord is

Head injury in an anticoagulated patient with drowsiness or vomiting suggests intracranial haemorrhage. Urgent CT, neurological observations, an INR and rapid reversal (for warfarin, prothrombin complex concentrate and IV vitamin K) are needed.

Antwoord

A · Urgent escalation for CT of the head and anticoagulant reversal

Antwoord: C · "As long as I take my tablets, I can swim alone."

Waarom dit het antwoord is

People with epilepsy should never swim alone, even if their seizures are well controlled. Showers are safer than baths. Telling friends about first aid and keeping a seizure diary are good practice.

Antwoord

C · "As long as I take my tablets, I can swim alone."

Antwoord: A · Radiation oesophagitis; analgesia, soft food and nutrition support

Waarom dit het antwoord is

Radiotherapy to the chest can inflame the oesophagus, causing painful swallowing. Analgesia (sometimes before meals), soft moist food, nutritional supplements, dietitian input and monitoring weight are important.

Antwoord

A · Radiation oesophagitis; analgesia, soft food and nutrition support
