1Formules en methoden

Prioriteren en delegeren

Methode

3Oplossingen

1.Prioriteren en delegeren

Antwoord: B · A parent of a 6-week-old with a temperature of 38,3 °C

Waarom dit het antwoord is

A fever in an infant under about 3 months old needs urgent assessment, because serious bacterial infection is more likely and signs can be subtle. The other calls are routine.

Antwoord

B · A parent of a 6-week-old with a temperature of 38,3 °C
2.Prioriteren en delegeren

Antwoord: A · Metabolic alkalosis

Waarom dit het antwoord is

Repeated vomiting of stomach contents loses hydrochloric acid, causing metabolic alkalosis, often with low chloride and potassium. Fluids and electrolytes are corrected before surgery.

Antwoord

A · Metabolic alkalosis
3.Infectiepreventie en patiëntveiligheid

Antwoord: C · Lock the wheels of both the bed and the stretcher before moving the patient

Waarom dit het antwoord is

Both surfaces are locked and at the same height before the transfer, a slide sheet is used and enough staff help. Side rails are raised on the stretcher during transport.

Antwoord

C · Lock the wheels of both the bed and the stretcher before moving the patient

Antwoord: A · About 30 minutes after the infusion ends

Waarom dit het antwoord is

Peak aminoglycoside levels are usually drawn about 30 minutes after a 30-minute infusion ends, to confirm the level is high enough to kill bacteria. Troughs, drawn just before the next dose, check for accumulation. Local protocols may differ, especially with once-daily dosing.

Antwoord

A · About 30 minutes after the infusion ends
6.Laboratoriumwaarden en elektrolyten

Antwoord: C · About 320 mOsm/kg, which is high

Waarom dit het antwoord is

Osmolality = 300 + 10 + 10 = 320 mOsm/kg, above the typical range of about 275–295 mOsm/kg. The high sodium is the main cause, which points to water loss or too little water intake.Gecontroleerd met: Osmolality blood test · MedlinePlus (NIH) (2025)

Antwoord

C · About 320 mOsm/kg, which is high
7.Klinische verpleegkunde

Antwoord: D · Repositioning and distraction

Waarom dit het antwoord is

Positioning, heat or cold where safe, relaxation, distraction and explanation can all reduce pain alongside analgesics.

Antwoord

D · Repositioning and distraction
8.Klinische verpleegkunde

Antwoord

B · Direct opschalen als neurologisch spoedgeval
9.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: D · Only while withdrawing the catheter

Waarom dit het antwoord is

The catheter is inserted without suction and suction is applied as it is withdrawn, with each pass kept short. This limits mucosal trauma and hypoxaemia. The patient is monitored for desaturation and bradycardia.

Antwoord

D · Only while withdrawing the catheter
10.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: B · Take blood cultures and give IV antibiotics within the first hour

Waarom dit het antwoord is

In sepsis with shock, blood cultures, IV antibiotics within the first hour, IV fluids, lactate measurement, oxygen if hypoxic and urine output monitoring are started together. Cultures should not delay antibiotics. Persistent hypotension may need vasopressors in critical care.

Antwoord

B · Take blood cultures and give IV antibiotics within the first hour
11.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: B · Beta-blockers can mask the tremor and racing heart of a hypo

Waarom dit het antwoord is

Beta-blockers blunt adrenergic warning symptoms such as palpitations and tremor. Sweating is often preserved. Patients should check their glucose when they feel unwell, rather than relying on symptoms alone.

Antwoord

B · Beta-blockers can mask the tremor and racing heart of a hypo

Antwoord: A · Explaining what will happen and answering questions

Waarom dit het antwoord is

Clear information about what to expect, including pain control and recovery, reduces anxiety and can improve recovery. False reassurance does not address the patient's concerns.

Antwoord

A · Explaining what will happen and answering questions
14.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: D · Pain may be undertreated; reassess and ask for a review of the plan

Waarom dit het antwoord is

Clock-watching often reflects inadequate analgesia. The nurse reassesses pain and function, considers multimodal options, and asks the prescriber to review the plan rather than labelling the patient.

Antwoord

D · Pain may be undertreated; reassess and ask for a review of the plan
