1Formules en methoden

Prioriteren en delegeren

Methode

3Oplossingen

1.Prioriteren en delegeren

Antwoord: B · “I will give my child aspirin to bring the fever down.”

Waarom dit het antwoord is

Aspirin in children with viral illnesses such as influenza or chickenpox is linked to Reye syndrome. Fluids, rest at home and reporting breathing trouble are correct.

Antwoord

B · “I will give my child aspirin to bring the fever down.”
2.Prioriteren en delegeren

Antwoord: B · A ruptured ectopic pregnancy

Waarom dit het antwoord is

A ruptured ectopic pregnancy causes internal bleeding, with abdominal pain, shoulder-tip pain and signs of shock. It is a surgical emergency.

Antwoord

B · A ruptured ectopic pregnancy
3.Infectiepreventie en patiëntveiligheid

Antwoord: B · Use latex-free supplies and schedule the patient as the first case of the day

Waarom dit het antwoord is

Latex proteins can become airborne on glove powder and settle in the room. Using a latex-safe room and supplies and scheduling the patient first, when airborne latex is lowest, reduce the risk.

Antwoord

B · Use latex-free supplies and schedule the patient as the first case of the day
4.Infectiepreventie en patiëntveiligheid

Antwoord: C · State the concern clearly a second time and, if still unresolved, use the chain of command

Waarom dit het antwoord is

The two-challenge rule says the concern is voiced at least twice to make sure it was heard. If it is still not addressed, the nurse escalates through the chain of command rather than acting against the patient’s safety.

Antwoord

C · State the concern clearly a second time and, if still unresolved, use the chain of command
5.Laboratoriumwaarden en elektrolyten

Antwoord: A · The result may be falsely high and should be repeated with better technique

Waarom dit het antwoord is

Repeated fist clenching and a long tourniquet time release potassium from muscle and can cause hemolysis, both of which give a falsely high potassium. The patient is assessed and the test is repeated with correct technique; true hyperkalemia must not be missed.

Antwoord

A · The result may be falsely high and should be repeated with better technique

7.Klinische verpleegkunde

Antwoord: D · With the head of the bed raised to at least 30 degrees

Waarom dit het antwoord is

Keeping the head raised to at least 30–45° during feeding and for a period afterwards lowers the risk of aspiration pneumonia.

Antwoord

D · With the head of the bed raised to at least 30 degrees
8.Klinische verpleegkunde

Antwoord: A · Thyroid storm

Waarom dit het antwoord is

Thyroid storm is a life-threatening worsening of hyperthyroidism, often triggered by infection or surgery. It needs urgent treatment with cooling, beta-blockade and antithyroid drugs.

Antwoord

A · Thyroid storm
9.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: C · Protect skin from sunlight and have thyroid, liver and lung checks

Waarom dit het antwoord is

Amiodarone causes photosensitivity, thyroid dysfunction (it contains iodine), liver toxicity, pulmonary fibrosis and corneal deposits. Patients use high-factor sunscreen and have regular thyroid and liver tests and reviews for cough or breathlessness. It also interacts with warfarin and digoxin.

Antwoord

C · Protect skin from sunlight and have thyroid, liver and lung checks
10.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: C · Unstable angina

Waarom dit het antwoord is

Unstable angina is new, worsening or rest angina without a troponin rise. It is an acute coronary syndrome and is managed urgently because it can progress to infarction. Stable angina is predictable and exertional. NSTEMI has a rising troponin.

Antwoord

C · Unstable angina
11.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: D · Check glucose before and after, and carry fast-acting carbohydrate

Waarom dit het antwoord is

Exercise increases glucose uptake and insulin sensitivity, so hypos can occur during and for hours after activity. Checking glucose, carrying carbohydrate and, if advised, adjusting insulin or food reduce the risk.

Antwoord

D · Check glucose before and after, and carry fast-acting carbohydrate
12.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: A · To reduce the risk of hypos, falls and confusion

Waarom dit het antwoord is

In frail older adults, tight control brings little long-term benefit and a high risk of hypoglycaemia, which can cause falls, fractures and confusion. Targets are individualised to balance safety and quality of life.

Antwoord

A · To reduce the risk of hypos, falls and confusion
13.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: A · Intraveneuze hydratatie met allopurinol of rasburicase

Antwoord

A · Intraveneuze hydratatie met allopurinol of rasburicase
14.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: D · Regular ibuprofen for back pain

Waarom dit het antwoord is

NSAIDs can cause kidney injury, fluid retention and GI bleeding in cirrhosis and should be avoided. Paracetamol at a reduced, carefully reviewed dose is usually preferred for pain.

Antwoord

D · Regular ibuprofen for back pain
