A patient with PTSD begins to dissociate during a conversation. Which grounding technique can the nurse use?

  1. AAsk her to close her eyes and think back to the worst moment of the trauma
  2. BAsk her to name things she can see, hear and touch in the room right now
  3. CLeave her alone until the episode has passed on its own
  4. DAsk her to describe the traumatic event in as much detail as she can

Two days after surgery, a 79-year-old woman is quiet and drowsy, stares into space and gives muddled answers that vary through the day. Her daughter says she was sharp before. What is most likely?

  1. ANormal tiredness after anaesthesia
  2. BHypoactive delirium
  3. CAdvanced dementia
  4. DDepression after surgery

How is adenosine given for supraventricular tachycardia?

  1. AAs a rapid IV push followed by a fast saline flush
  2. BAs an intramuscular injection into the outer thigh muscle
  3. CAs a slow infusion over one hour
  4. DAs a tablet under the tongue

A patient with diabetic ketoacidosis is about to start an IV insulin infusion. Serum potassium is 3,1 mmol/L (mEq/L). Why is this a concern?

  1. ALow potassium makes insulin ineffective, so a higher dose is needed
  2. BInsulin moves potassium into cells, so the level can fall dangerously low
  3. CPotassium has no link with insulin, so the infusion is started as planned
  4. DInsulin raises potassium further and can cause cardiac arrest, so the infusion is given faster

B · Ask her to name things she can see, hear and touch in the room right now

Grounding techniques use the senses to bring the person back to the here and now. Asking for details of the trauma during dissociation can deepen distress.

B · Ask her to name things she can see, hear and touch in the room right now

B · Hypoactive delirium

Hypoactive delirium presents with drowsiness, slowness and reduced activity, and is often missed or mistaken for depression. Acute onset, fluctuating course and inattention point to delirium.

B · Hypoactive delirium

A · As a rapid IV push followed by a fast saline flush

Adenosine must reach the heart quickly, so it is pushed rapidly into a large vein and flushed. Patients are warned about brief flushing, chest tightness and a short pause in the heartbeat.

A · As a rapid IV push followed by a fast saline flush

B · Insulin moves potassium into cells, so the level can fall dangerously low

Insulin drives potassium into cells, so a low starting level can become life-threatening hypokalaemia with arrhythmias. Potassium is replaced and monitored closely; the 2024 international consensus (ADA and others) delays insulin until potassium is above 3,5 mmol/L, while UK JBDS guidance adds potassium to the fluids and asks for senior review.

B · Insulin moves potassium into cells, so the level can fall dangerously low
