AOnly when the provider has written an order for a rapid response call in the chart
BOnly when the patient’s family makes a complaint
CWhen a patient shows an acute decline, such as new confusion or a sharp change in vital signs
DOnly after a patient has had a cardiac or respiratory arrest on the ward

Which patient should the nurse assess first?
AA patient on chemotherapy with a new temperature of 38,4 °C (101,1 °F)
BA patient with tuberculosis who is asking to have visitors
CA patient who wants more alcohol hand rub in the room
DA patient with an MRSA wound due for a routine dressing change

Why must a respirator such as an FFP3 or N95 mask be fit-tested?
AFit-testing checks whether the patient is still infectious
BFit-testing makes the mask reusable for a week
CIt must be tested to check it is the right colour
DIt only protects if it seals closely to the wearer's face

During tracheal suction a patient's SpO₂ falls from 95% to 85% and the heart rate drops. What should the nurse do?
AStop suctioning and give oxygen until the patient recovers
BIncrease the suction pressure to finish faster
CInsert the catheter further into the airway
DContinue until all the secretions have been removed from the airway

C · When a patient shows an acute decline, such as new confusion or a sharp change in vital signs
Rapid response teams bring critical care skills to a patient who is getting worse, with the aim of preventing cardiac arrest or ICU transfer. Any nurse who is worried about an acute change can call them.
C · When a patient shows an acute decline, such as new confusion or a sharp change in vital signs
A · A patient on chemotherapy with a new temperature of 38,4 °C (101,1 °F)
Fever in a patient who may be neutropenic can be the first sign of sepsis and needs urgent assessment. The other needs are real but stable and can wait a short time.
A · A patient on chemotherapy with a new temperature of 38,4 °C (101,1 °F)
D · It only protects if it seals closely to the wearer's face
A respirator filters tiny airborne particles only if air passes through the filter, not around a gap. Fit-testing and a fit-check every time it is worn make sure it seals.
D · It only protects if it seals closely to the wearer's face
A · Stop suctioning and give oxygen until the patient recovers
Suction removes oxygen and can stimulate the vagus nerve, causing hypoxia and bradycardia. The nurse stops, oxygenates and lets the patient recover before trying again.
A · Stop suctioning and give oxygen until the patient recovers
Відповідь: B · Гепатити A і E
