Before helping a patient from bed into a wheelchair, what should the nurse do with the wheelchair?
ALock the brakes and place it close to the bed on the patient’s stronger side
BPlace it on the patient’s weaker side so that side is supported
CLeave the brakes off so the chair can move along with the patient during the pivot
DPlace it at the foot of the bed, facing away
While preparing an IV medication, a nurse sees that the syringe tip touched the medication tray after the cap came off. What should the nurse do?
AGive the dose anyway, because the medication tray was cleaned this morning
BWipe the tip with an alcohol swab and give the dose
CBlow on the tip to remove any dust
DDiscard the syringe and prepare the dose again with new equipment
A patient can wash their face and arms but cannot reach their back and feet. What is the best approach?
ALet the patient do what they can and help with the rest
BWash the patient completely yourself to save time and effort
CLeave the back and feet unwashed
DAsk the patient to wait until a relative visits
A runner collapses on a hot day with a temperature of 40,5 °C and confusion. Why will paracetamol (acetaminophen) not help much?
AParacetamol needs a temperature below 37 °C to work
BThe heat comes from failed heat loss, not a raised set point
CParacetamol only lowers temperature in children, not in adults
DParacetamol makes the body produce more heat
AStreptococcus pneumoniae,
BLegionella pneumophila,
CMycoplasma pneumoniae,
DMycobacterium tuberculosis
A · Lock the brakes and place it close to the bed on the patient’s stronger side
Locked brakes stop the chair rolling away during the transfer. Placing it close to the bed on the stronger side lets the patient lead with their strong leg.
A · Lock the brakes and place it close to the bed on the patient’s stronger side
D · Discard the syringe and prepare the dose again with new equipment
In aseptic non-touch technique, key parts such as syringe tips and IV hubs must not touch any non-sterile surface. A contaminated key part is not cleaned and reused. It is replaced.
D · Discard the syringe and prepare the dose again with new equipment
A · Let the patient do what they can and help with the rest
Encouraging patients to do what they can maintains independence, dignity and function. The nurse helps with the parts they cannot manage.
A · Let the patient do what they can and help with the rest
B · The heat comes from failed heat loss, not a raised set point
In fever the hypothalamus raises the set point, and antipyretics lower it. In heatstroke the set point is normal but heat cannot be lost, so active cooling is needed.
B · The heat comes from failed heat loss, not a raised set point
D · Mycobacterium tuberculosis
