A patient with untreated hyperthyroidism has surgery and develops a temperature of 40 °C, a heart rate of 160/min and severe agitation. What is most likely happening?
A · Extra volume can quickly cause pulmonary oedema
In heart failure, the ventricle cannot cope with large increases in preload, so IV fluids can raise filling pressures and cause pulmonary oedema. When fluids are needed, smaller volumes are given with close reassessment of breathing, crackles, JVP, weight and fluid balance.
B · Treat the fever, pain and possible dehydration, then reassess
Sinus tachycardia is usually a physiological response to fever, pain, hypovolaemia, anxiety, hypoxia or sepsis. Treating the cause (antipyretics, analgesia, fluids, oxygen if needed) is appropriate, while watching for sepsis. Rate-slowing drugs can be harmful by masking compensation.
D · An oral anticoagulant
In AF the main cause of stroke is a clot from the left atrium. Oral anticoagulants such as a DOAC or warfarin prevent this far better than antiplatelets. The start date after a stroke depends on its size and bleeding risk.
B · Haloperidol
Haloperidol is a dopamine antagonist and can severely worsen parkinsonian symptoms. Metoclopramide is avoided for the same reason. Causes of delirium should be looked for, and specialist advice is needed if a drug is required.
D · Below 0.5 × 109/L
A neutrophil count below 0.5 × 109/L (500/mm³) is severe neutropenia, with a high risk of life-threatening infection. Fever in this setting is treated as neutropenic sepsis.
C · Use long-handled forceps to put it in the lead container; report it
A dislodged source is picked up with long-handled forceps and placed in the shielded container kept in the room. The radiation safety officer and team are informed, following local policy.
C · Cool it under cool running water for about 20 minutes
Cooling with running water for around 20 minutes limits tissue damage and pain. Ice can cause further injury, and the patient should be kept warm while the burn is cooled.
D · Not start until a correct identity band is in place and identity is confirmed
Positive patient identification, using an identity band checked against the blood unit and prescription, is essential before every transfusion. Wrong blood incidents can cause fatal ABO incompatibility reactions.
C · Primary hypothyroidism, which can raise lipids and lower sodium
A high TSH with a low free T4 is primary hypothyroidism. Low thyroid hormone reduces LDL clearance, raising cholesterol, and reduces free water excretion, which can cause hyponatremia. These often improve with levothyroxine.核对依据: Hypothyroidism - Medical Encyclopedia · MedlinePlus (NIH) (2024); TSH (Thyroid-stimulating hormone) Test · MedlinePlus (NIH) (2024); Osmolality blood test - Medical Encyclopedia · MedlinePlus (NIH) (2025)
B · Relieve pressure on the area by changing position regularly
Non-blanching redness is a stage 1 pressure injury. Relieving pressure through repositioning and support surfaces is the priority; massage can damage tissue further.
C · Thyroid storm
Thyroid storm is a life-threatening surge of thyroid hormone, with fever, tachycardia and agitation or confusion. It needs urgent treatment; myxedema coma is the opposite extreme of hypothyroidism.
