A · From the sampling port, using aseptic technique
Samples are taken from the needle-free sampling port after cleaning it. Urine in the bag is stale and contaminated, and breaking the system raises infection risk.
A · Notify the provider and start neurological observations, because a brain bleed may develop
Head injury in a patient on anticoagulants can cause bleeding inside the skull that shows signs later. The provider is told at once, imaging may be needed and neurological checks are done closely.
B · Turn the patient onto their side
A drowsy patient who vomits can aspirate. Turning them to the side lets vomit drain from the mouth; suction and a prescribed antiemetic follow.
A · Doing suctioning, turning and bathing all at the same time
Clustering several stimulating activities can cause a sustained rise in intracranial pressure. Spacing care, keeping the neck straight and a calm environment help keep it down.
A · Raise the concern through the local safeguarding (adult protection) procedure
Financial abuse is a form of adult abuse. Nurses report concerns through safeguarding procedures, document facts accurately and involve the patient as appropriate.
D · Arrange a professional interpreter
Professional interpreters provide accurate, impartial translation and protect confidentiality. Children and family members should not be used for important clinical discussions such as consent.
D · Ranges cover about 95% of healthy people, so some fall outside
A reference range is usually the central 95% of results from a healthy population, so about 1 in 20 healthy people will have a value just outside it. The result is interpreted with the clinical picture and previous values, and repeated if needed.
D · Platelets 70 × 10³/µL, creatinine doubled, bilirubin raised
In sepsis, falling platelets (coagulation), a rising creatinine (kidney) and a raised bilirubin (liver) show organ dysfunction, which indicates severe disease. The other sets are normal or near normal.
D · It lowers uric acid over time and does not relieve the pain of an acute attack
Allopurinol reduces urate production and is taken daily to prevent attacks. Acute attacks are treated with NSAIDs, colchicine or corticosteroids, and a new rash on allopurinol must be reported.
C · Stop or reduce the infusion as per protocol, lay the patient flat and notify the prescriber
Hypotension is a major adverse effect of nitrates due to venous and arterial dilation. The infusion is stopped or reduced, the patient is positioned flat to improve venous return, and the prescriber is informed.
C · Severe hypotension
Phosphodiesterase-5 inhibitors such as sildenafil enhance the vasodilator effect of nitrates and can cause profound, life-threatening hypotension. The combination is contraindicated, and patients should tell emergency staff if they have taken one.
C · Stop it at that site, inform the team and follow the extravasation policy
Extravasated noradrenaline causes intense local vasoconstriction and tissue necrosis. The infusion is stopped at that site (with a plan to continue elsewhere, since the patient depends on it), the team is informed and the local extravasation protocol is followed. Vasopressors are given through a central line where possible.
C · To help diagnose Cushing's syndrome
A dose of dexamethasone normally suppresses ACTH and cortisol. In Cushing's syndrome, cortisol stays high. The test is part of the work-up for cortisol excess.
A · A device that sends mild electrical pulses through skin pads
TENS is thought to reduce pain partly through the gate-control mechanism. It is non-invasive and patient-controlled, but is avoided over certain areas and with some implanted devices.
C · Hypoglycaemia if insulin continues without the feed
When feed stops but insulin continues, blood glucose can fall sharply. The insulin plan is reviewed with the team, glucose is checked frequently, and a glucose infusion may be needed.
B · Erythema toxicum
Erythema toxicum is a common, benign rash that appears in the first days and resolves without treatment. A well baby with a typical rash needs only reassurance, but blistering or an unwell baby needs review.
D · It is abnormal and needs prompt review
A score of 8 to 10 is normal, 6 is equivocal, and 4 or less is abnormal and suggests fetal hypoxia. At term, a low score usually leads to prompt review and consideration of birth.
B · Refer the baby to an eye specialist
An occasional squint is common in the first months, but a constant squint, or any squint after about 4–6 months, needs specialist assessment. Without treatment, the brain may suppress the image from the turning eye, causing amblyopia.
C · Fabricated or induced illness; follow safeguarding procedures
Fabricated or induced illness (formerly Munchausen syndrome by proxy) is a form of abuse in which a carer invents or causes illness. Careful objective documentation and early involvement of the safeguarding team are essential; the carer should not be confronted alone.
B · Protect him from ridicule, avoid arguing and focus on real activities
Arguing or agreeing strengthens a delusion. The nurse protects the patient's dignity, responds to his feelings and focuses on real, present activities and concerns.
R = 40 gtt/min
Infusion rates: a pump is set in mL/h = volume/time in hours. A gravity drip is counted in drops per minute: gtt/min = volume (mL) × drop factor (gtt/mL)/time (min). The drop factor is printed on the tubing package: macrodrip sets deliver 10, 15 or 20 gtt/mL (blood sets often 10 gtt/mL in the US, 15 in the UK) and microdrip sets 60 gtt/mL. Always use the drop factor the question or package gives.
使用微滴输液器(60 gtt/mL)时,gtt/min 等于 mL/h。
