A drowsy patient who has just returned from surgery starts to vomit. What should the nurse do first?
How should the urethral meatus be cared for in a patient with an indwelling urinary catheter?
A nurse calls a provider using SBAR about a patient with falling blood pressure. Which statement is the “Recommendation”?
After surgery for colon cancer, a patient's CEA had fallen to normal, but it has been rising over the last two checks. What does this most likely suggest?
A patient on IV gentamicin has a trough level above the target, and the creatinine has risen since yesterday. The next dose is due. What is the best action?
The fluid in an underwater seal chest drain has stopped swinging with breathing. What could this mean?
A pulse oximeter on a patient's finger gives an unreliable, flickering reading. Which factor is the most likely cause?
Twelve hours after a coronary stent, a patient has sudden severe chest pain again with new ST elevation in the same leads. What is the most likely cause?
Which group of problems is typical of primary hyperparathyroidism?
A 66-year-old smoker has had two episodes of painless red urine. Urine cultures are negative. Which plan is most appropriate?
A patient on modified-release morphine for cancer pain has needed four breakthrough doses every day for several days. What should the nurse do?
C · Turn the patient onto their side
A drowsy patient who vomits can aspirate. Turning them to the side lets vomit drain from the mouth; suction and a prescribed antiemetic follow.
B · With routine hygiene, using soap and water during daily washing
Routine perineal hygiene with soap and water is recommended. Antiseptic creams and routine irrigation do not reduce infection, and moving the catheter in and out carries organisms into the bladder.
D · “I think he needs to be assessed now. Can you come within the next 15 minutes?”
In SBAR, the situation names the problem, the background gives the history, the assessment shares the nurse’s findings and judgement, and the recommendation states a clear request with a time frame.
C · Possible recurrence that needs further assessment
CEA is not specific enough for screening, but a rising level after successful treatment can signal recurrence, so imaging and further assessment are usually arranged. Smoking and some benign conditions can also raise CEA.
D · Hold the dose and report the results to the prescriber
A high aminoglycoside trough with rising creatinine means the drug is accumulating, which increases the risk of kidney damage and hearing loss. The nurse holds the dose, reports to the prescriber and asks about hearing changes and tinnitus. Only the prescriber or pharmacist changes the regimen. Gentamicin sulfate injection - Boxed Warning (US prescribing information) · Pfizer Medical Information (FDA label)
D · The tube may be kinked or blocked, or the lung may have fully re-expanded
Loss of swing can mean a kinked, clamped or blocked tube, or re-expansion of the lung. The nurse checks the tubing and the patient and reports the finding.
A · Cold hands with poor peripheral circulation
Poor perfusion (cold, shock, vasoconstriction), movement, dark nail varnish or bright ambient light can make readings unreliable. The nurse checks the waveform, warms the hand or uses an ear probe, and compares the reading with the clinical picture.
D · Acute stent thrombosis
Acute stent thrombosis presents with recurrent ischaemic pain and ST elevation, and needs emergency return to the catheter laboratory. Missed antiplatelet doses are a major risk factor, which is why dual antiplatelet adherence is emphasised.
C · Kidney stones, bone pain, abdominal pain and low mood
High PTH raises calcium, which can cause kidney stones, bone loss and pain, constipation and abdominal pain, and low mood or confusion ("stones, bones, groans and moans"). Many cases are found on routine blood tests.
C · Urgent referral for cystoscopy and imaging
Painless visible haematuria in an older smoker, with no infection, must be investigated urgently for bladder or kidney cancer. Intermittent bleeding does not mean the cause has gone.
C · Report it, as the regular background dose may need review
Regular need for breakthrough doses usually means the background analgesia is inadequate. The prescriber reviews and may adjust the regular dose, while side effects such as sedation are monitored.
