On the first day after abdominal surgery, a patient has a temperature of 37.9 °C and reduced breath sounds at the lung bases. Which nursing action is most helpful?
An older adult has trouble getting in and out of the bath. Which measures make bathing safer?
Which patient with a suspected infection meets the qSOFA criteria that signal a higher risk of a poor outcome?
A patient's potassium is unexpectedly 5.8 mEq/L. The sample was taken after the tourniquet was on for several minutes while the patient pumped their fist. What is the best interpretation?
A patient has marked generalized edema. Results show heavy proteinuria, serum albumin 1.8 g/dL (18 g/L) and total cholesterol 340 mg/dL (8.8 mmol/L). Which condition fits best?
Which factor is most likely to slow wound healing?
A conscious patient with hypoglycaemia was given fast-acting carbohydrate. On re-check 15 minutes later the blood glucose is 3.6 mmol/L (65 mg/dL). What should the nurse do next?
Which history is a contraindication to thrombolysis for a STEMI?
A patient who has had a total laryngectomy becomes breathless and needs oxygen. How should it be given?
A patient with trigeminal neuralgia has lost weight and has poor oral hygiene. Which plan is most appropriate?
B · Encourage deep breathing, coughing and use of the incentive spirometer
Reduced basal breath sounds and a mild temperature early after surgery often mean atelectasis. Deep breathing, coughing, incentive spirometry and early mobilization help re-expand the lungs.
D · A shower chair, grab bars and a non-slip mat
Shower chairs, grab bars and non-slip surfaces reduce slips and falls. Bath oils make surfaces slippery, and a locked door delays help if the person falls.
B · Respiratory rate 24/min, new confusion, systolic BP 95 mmHg
qSOFA scores one point each for a respiratory rate of 22/min or more, altered mental status and a systolic BP of 100 mmHg or less. Two or more points mean higher risk, so the first patient needs urgent escalation.
A · The result may be falsely high and should be repeated with better technique
Repeated fist clenching and a long tourniquet time release potassium from muscle and can cause hemolysis, both of which give a falsely high potassium. The patient is assessed and the test is repeated with correct technique; true hyperkalemia must not be missed. Potassium Blood Test · MedlinePlus (NIH) (2024); Potassium test · MedlinePlus (NIH) (2025)
B · Nephrotic syndrome
Nephrotic syndrome is heavy proteinuria with low albumin, edema and high lipids. Low albumin reduces oncotic pressure, causing edema, and the liver increases lipoprotein production. There is also a higher risk of clots and infection. Nephrotic syndrome · MedlinePlus (NIH) (2025)
A · Smoking
Smoking causes vasoconstriction and reduces tissue oxygen, slowing healing and increasing infection risk. Poor nutrition, diabetes, infection and steroids also impair healing.
D · Give another dose of fast-acting carbohydrate and recheck
If blood glucose remains below the threshold (4 mmol/L in UK guidance, 70 mg/dL in US guidance), the fast-acting treatment is repeated. Once it is above the threshold, a longer-acting carbohydrate such as a sandwich or the next meal helps prevent recurrence.
C · A previous haemorrhagic stroke
Absolute contraindications include any previous intracranial haemorrhage, recent ischaemic stroke, active internal bleeding, suspected aortic dissection and recent major trauma or surgery. Thrombolysis is used when primary PCI cannot be delivered in time.
D · Via a mask or humidified device over the neck stoma
A total laryngectomy separates the trachea from the upper airway, so the patient breathes only through the neck stoma. Oxygen and any rescue breaths must be given via the stoma. These patients are "neck breathers" and often carry an alert card.
C · Offer soft, lukewarm food and advise chewing on the unaffected side
Patients may avoid eating and brushing their teeth because these trigger attacks. Soft, lukewarm food, chewing on the unaffected side, gentle oral care and effective drug treatment help maintain nutrition and hygiene.
