A nurse gives IV morphine for severe pain. When should the nurse reassess the pain and sedation?
A patient has full-thickness burns all the way around the chest and is finding it harder to breathe, although the airway is clear. What does this suggest?
A patient with delirium is at risk of falls and of pulling out lines. Which option is less restrictive than restraints?
An IV bag from the pharmacy holds the correct drug, but the label shows another patient’s name. What should the nurse do?
A patient taking spironolactone asks about using a salt substitute. What should the nurse explain?
A stable patient's hemoglobin has been 13–14 g/dL for days. Today's result is 8.2 g/dL, but the patient feels well, the vital signs are normal and there is no bleeding. What should the nurse suspect first?
Which group of symptoms suggests hypothyroidism?
Why must intravenous potassium chloride never be given as a rapid push or undiluted?
A patient with migraine takes painkillers or triptans on most days and now has a headache almost every day. What is the likely problem?
A patient has had a high sodium of 160 mmol/L for several days. Why must it be corrected slowly?
Which lung problem is a known toxicity of bleomycin?
Just after extubation in recovery, a patient develops a high-pitched noise on breathing in, with falling SpO₂. What should the nurse do first?
C · Within about 15–30 minutes
IV opioids act within minutes, so the nurse reassesses pain, sedation and breathing within about 15–30 minutes. Oral analgesics are usually reassessed later, after about an hour.
A · The tight burned skin is restricting chest movement, so an escharotomy may be needed
Circumferential full-thickness burns form stiff eschar that can stop the chest expanding. An escharotomy, cutting through the eschar, may be needed urgently to allow breathing.
B · A trained sitter or video monitoring with staff nearby
Close observation by a sitter or video monitoring allows staff to step in quickly, without restricting the patient. It is often combined with reorientation and family presence.
B · Not hang it, and get a correctly labeled bag from the pharmacy
A label with another patient’s name means the bag may be meant for someone else or may differ in other ways. The nurse does not use it, returns it and reports the issue so the error can be investigated.
B · Most salt substitutes contain potassium and can raise the level too high
Spironolactone blocks aldosterone, so the kidneys keep potassium. Many salt substitutes are made of potassium chloride, and combining them with spironolactone can cause dangerous hyperkalemia, especially with kidney disease or ACE inhibitors. Spironolactone · MedlinePlus (NIH) (2018); High potassium level · MedlinePlus (NIH) (2025)
C · A sample taken from the wrong patient
A sudden large change with no clinical change suggests a specimen error, such as wrong blood in the tube or dilution with IV fluid. The nurse assesses the patient, informs the provider and arranges a repeat sample with careful identification, while staying alert for true bleeding.
A · Tiredness, weight gain and feeling cold
Low thyroid hormone slows metabolism, causing fatigue, weight gain, cold intolerance, constipation, dry skin and bradycardia. Weight loss, heat intolerance and tremor suggest hyperthyroidism.
C · A sudden high potassium level can cause cardiac arrest
Rapid or concentrated IV potassium can cause cardiac arrest. It is given diluted, by infusion, at controlled rates (often with a pump and ECG monitoring for higher rates). Concentrated ampoules are tightly controlled in hospitals because of fatal errors.
D · Medication overuse headache
Frequent use of painkillers or triptans can cause medication overuse headache. Treatment involves withdrawing the overused drug under guidance and starting a preventive treatment if needed.
B · Rapid correction can cause brain swelling
In chronic hypernatraemia, brain cells adapt by holding extra solutes. If the sodium falls too quickly, water rushes into the cells and causes cerebral oedema and seizures. Correction is gradual, with regular sodium checks.
B · Pulmonary fibrosis
Bleomycin can cause pneumonitis and pulmonary fibrosis, especially at higher cumulative doses, in older patients and with high oxygen concentrations. Breathlessness or a new cough must be reported.
D · Call for help, give 100% oxygen and use a jaw thrust
Stridor after extubation suggests laryngospasm or another upper airway obstruction. The nurse calls for help, applies high-flow oxygen and airway manoeuvres, and the anaesthetist may give drugs or reintubate.
