MAP ≈ diastolic + (systolic − diastolic) ÷ 3 = 60 + 60 ÷ 3 = 80 mmHg. The heart spends longer in diastole, so MAP is closer to the diastolic value. A MAP of at least about 65 mmHg is a common target for organ perfusion.
B · 80 mmHg
C · Stop it at that site, inform the team and follow the extravasation policy
Extravasated noradrenaline causes intense local vasoconstriction and tissue necrosis. The infusion is stopped at that site (with a plan to continue elsewhere, since the patient depends on it), the team is informed and the local extravasation protocol is followed. Vasopressors are given through a central line where possible.
C · Stop it at that site, inform the team and follow the extravasation policy
Hypoglycaemia is a common complication of insulin–glucose treatment and can occur several hours later, especially in kidney failure. Glucose is checked regularly according to protocol, and potassium is rechecked.
D · Capillary glucose, for hypoglycaemia
A · A partial-thickness burn on the hand
Burns of the face, hands, feet, genitalia, perineum and major joints, as well as circumferential, chemical, electrical and inhalation injuries, usually need specialist advice because of their risks and functional impact.
A · A partial-thickness burn on the hand
B · That the catheter and tubing are not kinked or blocked
A sudden low output may be due to a kinked tube or blocked catheter. If drainage is normal, hypovolaemia or kidney injury is considered and the team is informed, with a fluid assessment.
B · That the catheter and tubing are not kinked or blocked
C · The letters can be misread as a zero, causing a tenfold overdose
"U" can be misread as 0, so "10U" might be read as 100, and "IU" can be mistaken for "IV" or "10". Writing "units" in full is a standard safety rule (ISMP), because insulin errors can cause severe hypoglycaemia.
C · The letters can be misread as a zero, causing a tenfold overdose
B · Potassium, because trimethoprim can raise it
Trimethoprim blocks sodium channels in the collecting duct, reducing potassium excretion, much like amiloride. Hyperkalemia is more likely in older adults, with kidney disease, or with ACE inhibitors, ARBs or spironolactone.
B · Potassium, because trimethoprim can raise it
B · Cushing syndrome
Excess cortisol raises glucose, causes central obesity, a round face and purple striae, and has some mineralocorticoid effect that lowers potassium. Adrenal insufficiency causes low glucose and high potassium.
B · Cushing syndrome
Osm = 338 mOsm/kg
ضغط الأكسجين السنخي هو PAO₂ = FiO₂·(760 − 47) − PaCO₂/0,8 mmHg عند مستوى البحر، والفرق A–a هو PAO₂ − PaO₂.
Osm = 2·152 + 18,5 + 15,6 = 338,1 ≈ 338 mOsm/kg
Osm = 338 mOsm/kg
الإجابة: الفرق A–a = 72,14 mmHg
ضغط الأكسجين السنخي هو PAO₂ = FiO₂·(760 − 47) − PaCO₂/0,8 mmHg عند مستوى البحر، والفرق A–a هو PAO₂ − PaO₂.
توقف = 40 × 0,04 s = 1,6 s
توقف = 1,6 s

C · A patient with stable cellulitis on scheduled IV antibiotics
A newly oriented nurse is best given stable patients with predictable care. The other patients have changing or high-risk conditions that need more experience and close monitoring.
C · A patient with stable cellulitis on scheduled IV antibiotics
C · The confused patient climbing out of bed
A confused patient climbing out of bed could fall and be badly injured within moments. Pain needs prompt treatment, but the immediate safety risk comes first.
