C · Diphenhydramine
B · Check two identifiers against the wristband and the medication record
The nurse uses at least two identifiers, such as full name and date of birth, checked against the wristband and record. Open questions (“Please tell me your name”) are safer than yes/no questions.
B · Check two identifiers against the wristband and the medication record
D · The lump has become painful and tender and cannot be pushed back
An irreducible, painful, tender hernia, especially with vomiting, suggests incarceration or strangulation. This is a surgical emergency because the trapped bowel can become ischaemic.
D · The lump has become painful and tender and cannot be pushed back
A · Lowering the sodium too fast can cause cerebral oedema
In chronic hypernatraemia, brain cells adapt by holding extra solutes. If sodium falls too quickly, water moves into brain cells and causes cerebral oedema, so correction is controlled.
A · Lowering the sodium too fast can cause cerebral oedema
A · The drug and its metabolites build up and cause long lows
Glyburide and its active metabolites are cleared by the kidneys, so they accumulate in CKD and can cause severe, prolonged hypoglycemia. Older adults are at greater risk. The prescriber is informed, and a different drug is usually chosen.
A · The drug and its metabolites build up and cause long lows
B · Calcium gluconate
Absent reflexes and slow breathing indicate magnesium toxicity. The infusion is stopped, the provider is called, the airway and breathing are supported, and IV calcium gluconate is given as prescribed to counteract magnesium.
B · Calcium gluconate
D · Limit coffee, tea and cola, because caffeine adds to the side effects
Theophylline has a narrow therapeutic range, and caffeine adds stimulant effects such as tachycardia, restlessness and insomnia. Toxicity can cause arrhythmias and seizures, so blood levels may be monitored.
D · Limit coffee, tea and cola, because caffeine adds to the side effects
C · Water intoxication from the antidiuretic effect of oxytocin
Oxytocin has antidiuretic activity at high doses. With large volumes of fluid this can cause water retention and dilutional hyponatraemia, so fluid balance and sodium are monitored.
C · Water intoxication from the antidiuretic effect of oxytocin
C · Pneumothorax
Subclavian puncture can injure the pleura and cause a pneumothorax. A chest X-ray is usually taken after implantation. Other complications include haematoma, lead displacement and infection.
C · Pneumothorax

Bleeding is the main complication of kidney biopsy. The patient usually has bed rest for some hours, with frequent BP and pulse checks and inspection of urine for haematuria. Back pain or a falling BP is reported at once.
A · Signs of bleeding, such as haematuria and a rising pulse
A · Paralytic ileus, so keep nil by mouth and prepare for an NG tube
Paralytic ileus is common in the first days after a spinal cord injury. The patient is kept nil by mouth, and a nasogastric tube decompresses the stomach to prevent vomiting and aspiration until bowel sounds return.
A · Paralytic ileus, so keep nil by mouth and prepare for an NG tube
D · IV fluids followed by an IV bisphosphonate
Hypercalcaemia causes polyuria and dehydration, so IV saline is given first, then a bisphosphonate such as zoledronic acid to reduce bone resorption. Calcium, kidney function and symptoms are monitored.
D · IV fluids followed by an IV bisphosphonate
C · Capillary blood glucose

A · Apply gentle pressure to the head to slow the birth
Gentle pressure on the fetal head, with the woman panting, slows birth and reduces perineal tearing and rapid pressure changes in the fetal skull. The nurse then checks for a nuchal cord and supports the baby's body.
A · Apply gentle pressure to the head to slow the birth
C · Oral rehydration solution
Oral rehydration solution has the right balance of sodium and glucose to promote water absorption. Juice, fizzy drinks and many sports drinks contain too much sugar and too little sodium and can worsen diarrhoea.
C · Oral rehydration solution
D · Poor weight gain, refusing feeds and blood in the vomit

Body-surface-area dosing: many cytotoxic and some paediatric doses are prescribed per square metre of body surface area: dose (mg) = dose per m² × BSA (m²). The Mosteller formula estimates BSA = √(height (cm) × weight (kg)/3600).
BSA = √(190·92/3600) = √4,856 = 2,204 m²
D = 60·2,204 = 132,2 mg
