D · Hold the bottle with the label in the palm and pour without touching the bowl

B · State the concern clearly a second time and, if still unresolved, use the chain of command

The two-challenge rule says the concern is voiced at least twice to make sure it was heard. If it is still not addressed, the nurse escalates through the chain of command rather than acting against the patient’s safety.

B · State the concern clearly a second time and, if still unresolved, use the chain of command

A · “I add extra salt to my food so it tastes better.”

B · A patient who has just become breathless after starting an IV antibiotic

D · Potassium and creatinine

ACE inhibitors reduce aldosterone, so potassium can rise, and they lower the filtration pressure in the kidney, so creatinine can rise. Both are checked before and soon after starting or increasing the dose. Hyperkalemia · StatPearls (NCBI Bookshelf) (2025)

D · Potassium and creatinine

A · A low erythropoietin level

In polycythemia vera, a marrow mutation (usually JAK2) drives red cell production, so erythropoietin is low. In secondary polycythemia from hypoxia (COPD, high altitude, smoking), erythropoietin is high. Often white cells and platelets are also raised in polycythemia vera. Erythropoietin test · MedlinePlus (NIH) (2025); Polycythemia vera · MedlinePlus (NIH) (2025)

A · A low erythropoietin level

A · Buccal or intranasal midazolam

Without IV access, buccal or intranasal midazolam, or rectal diazepam, is used to stop a prolonged seizure. Nothing is put into the mouth to swallow during a seizure.

A · Buccal or intranasal midazolam

C · Check the patient and the electrodes, as this is likely artefact

Movement, loose electrodes and muscle activity can create artefact that looks like VF. A conscious, talking patient cannot be in VF. The nurse assesses the patient and checks the leads and electrodes, then reattaches them if needed.

C · Check the patient and the electrodes, as this is likely artefact

A · Stop the PCA, support breathing, give oxygen and get naloxone

Sedation and slow breathing are warning signs of opioid toxicity. The nurse stops the opioid, stimulates the patient, opens the airway, gives oxygen, calls for help and gives naloxone per protocol. Naloxone is short-acting, so the patient is monitored for recurrence.

A · Stop the PCA, support breathing, give oxygen and get naloxone

C · The injury is incomplete, which suggests a better chance of recovery

Preserved sensation or movement in the lowest sacral segments (sacral sparing) means the injury is incomplete. Incomplete injuries have a much better chance of some functional recovery.

C · The injury is incomplete, which suggests a better chance of recovery

B · Suspect aspiration pneumonia, stop oral intake and escalate for review

These signs suggest aspiration pneumonia, which is a common and serious complication of dysphagia. The patient needs medical review, chest assessment, oxygen if needed, and reassessment of their swallow before eating again.

B · Suspect aspiration pneumonia, stop oral intake and escalate for review

C · Hourly urine output guides fluid resuscitation

x = 36 gtt/min

x = 36 gtt/min

00:00 → 06:00 = 360 min

t = 75 + 360 = 435 min

t = 435 min

الإجابة: V = 0,3 mL

Solution strengths: a percentage strength is grams per 100 mL, so 1% = 1 g/100 mL = 10 mg/mL. A ratio strength 1:1000 means 1 g in 1000 mL = 1 mg/mL; 1:10 000 is 0,1 mg/mL. For a dilution the amount of drug stays the same: C₁·V₁ = C₂·V₂.

1:1000 = 1 g/1000 mL = 1 mg/mL

V = 0,3/1 = 0,3 mL

V = 0,3 mL

الإجابة: D = 74,69 mg

Body-surface-area dosing: many cytotoxic and some paediatric doses are prescribed per square metre of body surface area: dose (mg) = dose per m² × BSA (m²). The Mosteller formula estimates BSA = √(height (cm) × weight (kg)/3600).

BSA = √(119·30/3600) = √0,9917 = 0,9958 m²

D = 75·0,9958 = 74,69 mg

D = 74,69 mg

Infusion rates: a pump is set in mL/h = volume/time in hours. A gravity drip is counted in drops per minute: gtt/min = volume (mL) × drop factor (gtt/mL)/time (min). The drop factor is printed on the tubing package: macrodrip sets deliver 10, 15 or 20 gtt/mL (blood sets often 10 gtt/mL in the US, 15 in the UK) and microdrip sets 60 gtt/mL. Always use the drop factor the question or package gives.

1000/8 = 125 mL/h

R = 960/6 = 160 mL/h

R = 160 mL/h

D = 10 mcg/min

Dose-based infusions: concentration = amount of drug in the bag/bag volume. Pump rate (mL/h) = dose per hour/concentration, and dose per hour = mL/h × concentration. Convert per-minute doses to per-hour (× 60) and keep the mass units the same (1 g = 1000 mg, 1 mg = 1000 mcg).

50000 mcg/250 mL = 200 mcg/mL

D = 600/60 = 10 mcg/min

D = 10 mcg/min

الإجابة: I = 920 mL

I = 920 mL

I = 920 mL

75·2 = 150 mL
500 − 150 = 350 mL المتبقي

350/60 = 5,833 h

t = 2 + 5,833 = 7,833 ≈ 7,8 h

t = 7,8 h
