C · Give five rescue breaths; if breathing starts, tag the child red

In JumpSTART, an apneic child with a pulse gets five rescue breaths, because the cause is often respiratory. If breathing starts the child is immediate (red); if not, the child is expectant.

C · Give five rescue breaths; if breathing starts, tag the child red

C · Hold the bottle with the label in the palm and pour without touching the bowl

The nurse pours from a little height without touching the sterile bowl, avoiding splashes that would wet the field. Holding the label in the palm keeps it readable and free of drips.

C · Hold the bottle with the label in the palm and pour without touching the bowl

C · Stop the infusion and check the patient’s blood glucose

A concentration error means the dose delivered was wrong. The nurse stops the infusion, assesses the patient including blood glucose, treats as needed, notifies the prescriber and reports the event.

C · Stop the infusion and check the patient’s blood glucose

C · Kidney function (creatinine clearance)

Dabigatran is excreted mainly by the kidneys, so poor kidney function raises the drug level and bleeding risk. Kidney function is checked before starting and regularly afterwards. Routine INR monitoring is not used for direct oral anticoagulants.

C · Kidney function (creatinine clearance)

B · Thrombotic thrombocytopenic purpura (TTP)

TTP causes small clots that consume platelets, break red cells (microangiopathic hemolysis) and damage the brain and kidneys, while clotting times usually stay normal. It is an emergency treated with plasma exchange; platelet transfusion is generally avoided.

B · Thrombotic thrombocytopenic purpura (TTP)

A · An air leak, from the lung or from the drainage system

Bubbling with breathing in a drain for pneumothorax shows air is leaving the pleural space. Continuous bubbling can also mean a leak in the connections, so the system is checked and the finding reported.

A · An air leak, from the lung or from the drainage system

D · Its active metabolites build up because the kidneys excrete them

Morphine metabolites, including morphine-6-glucuronide, are cleared by the kidneys and accumulate in renal impairment, causing sedation and respiratory depression. Alternative opioids or lower doses are often chosen.

D · Its active metabolites build up because the kidneys excrete them

B · Too high a pressure damages the trachea; too low allows aspiration

Cuff pressure is usually kept at about 20–30 cmH₂O. Higher pressures reduce blood flow to the tracheal mucosa, risking ulceration and stenosis; lower pressures allow secretions to leak past and cause aspiration and VAP. A cuff manometer is used.

B · Too high a pressure damages the trachea; too low allows aspiration

D · Changes in mood, such as irritability, low mood or anger

Levetiracetam is widely used and has few drug interactions, but it can cause mood changes, irritability, depression and, rarely, suicidal thoughts. Patients and families should report these. Gum overgrowth is linked to phenytoin.

D · Changes in mood, such as irritability, low mood or anger

B · Autonomic dysfunction, which needs continuous cardiac monitoring

Guillain–Barré syndrome can involve autonomic nerves, causing labile blood pressure and dangerous arrhythmias. Patients need continuous ECG monitoring and close observation, often in a high-dependency setting.

B · Autonomic dysfunction, which needs continuous cardiac monitoring

C · Clamp the line, lie the patient left side down, head low; give oxygen

Air can enter an open central line. Clamping stops more air entering; the left lateral, head-down position may keep air in the right heart away from the pulmonary outflow. High-flow oxygen and emergency help are given.

C · Clamp the line, lie the patient left side down, head low; give oxygen
