B · Hold the next dose and report for an urgent lithium level

Vomiting, diarrhea and a coarse tremor suggest lithium toxicity. The nurse holds the dose and reports so a serum level can be checked; low salt or fluid intake raises lithium levels further.

B · Hold the next dose and report for an urgent lithium level

C · Large-volume IV fluids with close monitoring of urine output

Once the airway is secure, the early priority in major burns is fluid resuscitation to prevent burn shock. Urine output is a key guide to whether enough fluid is being given.

C · Large-volume IV fluids with close monitoring of urine output

B · The drug can mask warning signs of hypoglycemia

Beta-blockers, especially nonselective ones, blunt adrenergic warning signs of hypoglycemia such as tremor and tachycardia, and they can delay recovery. Sweating usually remains. Glucose checks help detect lows that the patient may not feel.

B · The drug can mask warning signs of hypoglycemia

C · "Dried apricots and raisins will be a good snack for me now"

Dried fruits such as apricots and raisins are very high in potassium and should be limited in kidney disease with hyperkalemia. Limiting high-potassium foods, leaching vegetables and reading labels are correct strategies.

C · "Dried apricots and raisins will be a good snack for me now"

C · Stopping early greatly increases the risk of a clot forming in the stent

Dual antiplatelet therapy prevents stent thrombosis while the stent becomes covered by the artery lining. Stopping early can cause a sudden myocardial infarction.

C · Stopping early greatly increases the risk of a clot forming in the stent

B · "A big meal just before bed is fine if I take my tablet."

Large late meals increase reflux even when a PPI is taken. Raising the head of the bed, losing weight and reducing alcohol are correct measures.

B · "A big meal just before bed is fine if I take my tablet."

D · Spare tubes (same size and one smaller), suction and oxygen

Bedside equipment usually includes spare tracheostomy tubes (same size and one size smaller), an obturator if used, suction with catheters, oxygen, a bag-valve-mask and tracheal dilators where policy requires. It is checked at every shift.

D · Spare tubes (same size and one smaller), suction and oxygen

B · Report it so occupational health can arrange follow-up screening

Contact tracing includes staff exposed without respiratory protection. Occupational health assesses the risk, checks for symptoms and arranges testing (for example IGRA) at suitable intervals. Treatment is offered for latent infection if found. Exposed people do not usually need to stop work unless they become unwell.

B · Report it so occupational health can arrange follow-up screening

A · Glucose absorbed from the dialysis fluid

Standard PD fluids contain glucose to pull water out by osmosis. Some glucose is absorbed, adding calories and raising blood glucose. Glucose monitoring and diet advice are needed, and other solutions may be used.

A · Glucose absorbed from the dialysis fluid

A · Emptying the pouch and measuring the output

Emptying and measuring output is a routine task. Assessment of the stoma, teaching and decisions about treatment remain with the registered nurse, who reviews the recorded output.

A · Emptying the pouch and measuring the output

C · A non-smoking woman with motion sickness who will need opioids

Apfel risk factors are female sex, non-smoking status, a history of PONV or motion sickness, and postoperative opioids. Patients with more factors receive more preventive antiemetics.

C · A non-smoking woman with motion sickness who will need opioids
