B · Potassium chloride by IV push

Potassium chloride must never be given by IV push, because a rapid dose can cause cardiac arrest. It is diluted and given slowly through an infusion pump.

B · Potassium chloride by IV push

C · Hyperosmolar hyperglycemic state

Hyperosmolar hyperglycemic state occurs mainly in type 2 diabetes, with extreme glucose, severe dehydration and little or no ketosis. Treatment starts with IV fluids, as in DKA.

C · Hyperosmolar hyperglycemic state

C · Remove items that could be used for self-harm, such as belts and cords

Patients at risk of suicide need a safe environment, with belts, cords, sharp objects and glass removed, and observation at the level ordered. Privacy is secondary to safety in this situation.

C · Remove items that could be used for self-harm, such as belts and cords

B · Glucagon by injection, or IV dextrose as prescribed

A patient who cannot swallow safely must not receive anything by mouth because of the risk of aspiration. Glucagon (injected or intranasal) or IV dextrose raises the glucose; once awake and able to swallow, the patient is given oral carbohydrate and a snack. Hypoglycemia · MedlinePlus (NIH); Low blood sugar · MedlinePlus (NIH) (2026)

B · Glucagon by injection, or IV dextrose as prescribed

C · G6PD deficiency with acute hemolysis

G6PD deficiency makes red cells vulnerable to oxidative damage. Fava beans, some drugs (such as primaquine and some sulfonamides) and infections can trigger acute hemolysis, with dark urine, jaundice and anemia. Triggers are avoided.

C · G6PD deficiency with acute hemolysis

B · Respiratory acidosis

A raised PaCO2 with a low pH means the lungs are not removing enough CO2, for example from hypoventilation. A normal bicarbonate suggests it is acute.

B · Respiratory acidosis

D · Lying flat or slightly head-down, holding the breath or breathing out

A flat or head-down position and a Valsalva manoeuvre or breath-hold raise central venous pressure, reducing the risk of air being drawn in. An occlusive dressing is then applied.

D · Lying flat or slightly head-down, holding the breath or breathing out

D · Low potassium increases the risk of digoxin toxicity

Digoxin binds to the Na⁺/K⁺-ATPase pump at the site potassium normally uses. When potassium is low, more digoxin binds, so toxicity and arrhythmias become more likely. Loop diuretics commonly cause hypokalaemia, so the combination needs monitoring.

D · Low potassium increases the risk of digoxin toxicity

C · Stop smoking and do not fly until a doctor confirms full resolution

Patients should not fly until the pneumothorax has resolved on X-ray, often with an extra waiting period. Diving is usually avoided permanently unless definitive surgery has been done. Smoking increases recurrence, which is common, so patients learn to seek help for new breathlessness or chest pain.

C · Stop smoking and do not fly until a doctor confirms full resolution

A · "I will hold my urine for as long as I can."

Holding urine allows bacteria to multiply. Regular voiding, good fluid intake, passing urine after intercourse and wiping front to back help reduce recurrence.

A · "I will hold my urine for as long as I can."

D · A partial-thickness burn becoming deeper, with new discolouration

Signs of burn wound infection include conversion to a deeper burn, new dark or green discolouration, increasing pain, offensive exudate, spreading redness and systemic signs such as fever or tachycardia.

D · A partial-thickness burn becoming deeper, with new discolouration

C · Report it, keep it clean and use a non-adherent dressing

Moist desquamation is a more severe radiation skin reaction. The radiotherapy team is informed; the area is kept clean and covered with non-adherent dressings, avoiding adhesive on the skin, and analgesia is given. Infection is watched for.

C · Report it, keep it clean and use a non-adherent dressing
