C · Increasing sleepiness that is hard to rouse

Sedation usually increases before breathing slows. Regular sedation checks let the nurse hold the opioid and act before respiratory depression occurs.

C · Increasing sleepiness that is hard to rouse

C · Check that a swallow screen has been done before giving anything by mouth

After a stroke, the patient stays NPO (nothing by mouth) until a swallow screen shows it is safe. Giving fluids first risks aspiration, even with a straw or thickener.

C · Check that a swallow screen has been done before giving anything by mouth

B · Around the patient’s waist, over clothing

The gait belt goes snugly around the waist over clothing, close to the center of gravity, with room for the nurse’s fingers underneath. Holding it gives control without pulling on the arms.

B · Around the patient’s waist, over clothing

C · There are too few neutrophils to produce the usual inflammatory signs

Classic signs of infection come from the inflammatory response, which depends on white blood cells. With very few neutrophils, fever may be the only early sign, so it must be taken seriously.

C · There are too few neutrophils to produce the usual inflammatory signs

C · Lifelong vitamin B12 replacement

Intrinsic factor from the stomach is needed to absorb vitamin B12 in the ileum. After a total gastrectomy, B12 must be replaced for life, usually by injection, to prevent macrocytic anemia and nerve damage. Vitamin B12 or folate deficiency anaemia - Causes · NHS (2023); Vitamin B12 deficiency anemia · MedlinePlus (NIH) (2026)

C · Lifelong vitamin B12 replacement

A · Persistent bacteremia, as in infective endocarditis

Several positive cultures with the same organism, taken at different times, show continuous bacteremia, typical of infective endocarditis. Echocardiography is done and prolonged IV antibiotics are started. Contamination usually affects only one bottle. Blood culture · MedlinePlus (NIH) (2025)

A · Persistent bacteremia, as in infective endocarditis

A · Moist but not wet

A moist wound environment speeds epithelial cell migration and healing. Too much moisture causes maceration, so dressings are chosen to manage exudate.

A · Moist but not wet

B · A ruptured abdominal aortic aneurysm

A ruptured AAA classically presents with pain, shock and a pulsatile mass. It is a surgical emergency needing immediate senior help and transfer to theatre.

B · A ruptured abdominal aortic aneurysm

D · When the gag and cough reflexes have returned

Local anaesthetic sprayed on the throat abolishes the gag and cough reflexes for a time, so food or fluid could be aspirated. The nurse checks that the reflexes have returned (often after about one to two hours) before giving sips of water and then food.

D · When the gag and cough reflexes have returned

D · Directly observed therapy with support for barriers to adherence

Directly observed therapy, where a health worker watches each dose, plus help with practical and social barriers, improves completion and reduces multidrug-resistant TB. Single-drug therapy or interrupted treatment breeds resistance.

D · Directly observed therapy with support for barriers to adherence

C · They prolong the action of gut incretin hormones

DPP-4 inhibitors slow the breakdown of incretins such as GLP-1, so more insulin is released and less glucagon after meals. They rarely cause hypoglycaemia on their own and are weight-neutral.

C · They prolong the action of gut incretin hormones

D · A non-smoking woman with motion sickness who will need opioids

Apfel risk factors are female sex, non-smoking status, a history of PONV or motion sickness, and postoperative opioids. Patients with more factors receive more preventive antiemetics.

D · A non-smoking woman with motion sickness who will need opioids
