C · Stop the infusion and remove the cannula

A cool, pale, swollen site suggests infiltration. The nurse stops the infusion, removes the cannula, elevates the limb and restarts the IV at another site as needed.

C · Stop the infusion and remove the cannula

C · Serum phosphate

In refeeding syndrome, a sudden change to carbohydrate metabolism moves phosphate, potassium and magnesium into cells. Low phosphate can cause weakness, confusion, heart failure and breathing problems.

C · Serum phosphate

A · A low night light that lights the path to the bathroom

A night light lets the patient see obstacles and the way to the bathroom without disturbing sleep. Many falls happen in the dark on the way to the toilet.

A · A low night light that lights the path to the bathroom

C · From the right arm

Venipuncture, blood pressure readings and IV lines are avoided in the arm with an AV fistula or graft, to prevent damage, clotting and infection of the access. Only the dialysis team uses the access, for dialysis.

C · From the right arm

الإجابة: A · 450/µL

ANC = 1,800 × (0,20 + 0,05) = 450/µL. A value below 500/µL is severe neutropenia with a high risk of serious infection, so neutropenic precautions and early reporting of fever are essential. Blood Differential · MedlinePlus (NIH) (2024)

A · 450/µL

D · "I will only use it when I'm wheezy."

Inhaled corticosteroids must be used regularly to reduce airway inflammation and prevent attacks. The short-acting reliever is used for symptoms.

D · "I will only use it when I'm wheezy."

A · Raised blood glucose

Corticosteroids raise blood glucose, which matters especially in diabetes. Short courses can also cause mood changes, insomnia, indigestion and fluid retention. Long-term use adds osteoporosis, adrenal suppression and infection risk.

A · Raised blood glucose

C · Syndrome of inappropriate ADH secretion

Small-cell lung cancer can secrete ADH, causing water retention and dilutional hyponatraemia (normal sodium about 135–145 mmol/L). Urine is inappropriately concentrated and the patient is not dry. Fluid restriction and treating the cancer are the main measures; sodium is corrected slowly to avoid osmotic demyelination.

C · Syndrome of inappropriate ADH secretion

C · Preventive antibiotics

Close contacts, such as people living in the same household, are offered chemoprophylaxis, for example with ciprofloxacin or rifampicin (rifampin), to clear carriage. Public health teams usually coordinate this.

C · Preventive antibiotics

A · Use healthy sites and check glucose often, as less insulin may be needed

Lipohypertrophy causes poor and unpredictable absorption. When the patient switches to healthy tissue, more insulin is absorbed, so hypos can occur. Glucose is monitored closely, and doses may need to be reduced.

A · Use healthy sites and check glucose often, as less insulin may be needed

B · Normal protein and enough calories, with a late-evening snack

Protein restriction worsens malnutrition and muscle loss and is no longer routinely advised. Adequate protein and energy, small frequent meals and a late-evening snack are recommended.

B · Normal protein and enough calories, with a late-evening snack

B · An intramuscular injection

IM injections can cause haematomas in severe thrombocytopenia. Rectal procedures, NSAIDs and aspirin are also avoided. The nurse checks with the prescriber and the oral or IV route is used where possible.

B · An intramuscular injection
