C · “I will swallow the tablet with a glass of water.”
Sublingual tablets dissolve under the tongue and are absorbed quickly there. Sitting down reduces falls from low blood pressure, the tablets are protected from light and moisture, and headache is a common side effect.
C · “I will swallow the tablet with a glass of water.”
A · Diabetes insipidus
Damage to the pituitary or hypothalamus can stop the release of antidiuretic hormone, causing diabetes insipidus. The patient loses water, so urine is dilute and sodium rises; SIADH causes the opposite.
A · Diabetes insipidus
D · High glucose impairs white cell function and raises the risk of surgical site infection
Hyperglycemia weakens neutrophil function and slows healing, so it increases the risk of surgical site infection. Keeping glucose in the target range is part of infection prevention for surgical patients.
D · High glucose impairs white cell function and raises the risk of surgical site infection
D · Lying face down (prone)
Restraining a person face down can restrict breathing and has caused deaths from positional asphyxia. Restrained patients are positioned to protect the airway and are watched closely.
D · Lying face down (prone)
B · It falls, because GFR rises
In pregnancy, kidney blood flow and GFR rise by about 50%, so creatinine falls below non-pregnant values. A creatinine that looks "normal" for a non-pregnant adult may indicate reduced kidney function in pregnancy.
B · It falls, because GFR rises
B · Urgent dialysis
Indications for urgent dialysis in AKI include refractory hyperkalemia, severe metabolic acidosis, fluid overload not responding to diuretics, uremic complications and some poisonings. This patient has three of them. Acute tubular necrosis · MedlinePlus (NIH) (2025)
B · Urgent dialysis
D · Unopened insulin is kept in the fridge and must not be frozen
Unopened insulin is refrigerated; in-use pens or vials are usually kept at room temperature for a limited time stated by the manufacturer. Frozen or overheated insulin must be discarded.
D · Unopened insulin is kept in the fridge and must not be frozen
C · A person is not treated as lacking capacity just because they make an unwise decision
One of the Act's principles is that making an unwise decision is not, on its own, evidence of incapacity. The team should check understanding, explore reasons and document the discussion.
C · A person is not treated as lacking capacity just because they make an unwise decision
A · Cold hands with poor peripheral circulation
Poor perfusion (cold, shock, vasoconstriction), movement, dark nail varnish or bright ambient light can make readings unreliable. The nurse checks the waveform, warms the hand or uses an ear probe, and compares the reading with the clinical picture.
A · Cold hands with poor peripheral circulation
C · "I will use a salt substitute instead of ordinary salt."
Many salt substitutes contain potassium chloride. With an ACE inhibitor and spironolactone, both of which raise potassium, they can cause dangerous hyperkalaemia. Daily weights, reporting rapid weight gain and avoiding NSAIDs (which cause fluid retention) are correct.
C · "I will use a salt substitute instead of ordinary salt."
A · The parathyroid glands may have been damaged, lowering calcium
The parathyroid glands lie behind the thyroid and can be damaged or removed during surgery. Low PTH leads to hypocalcaemia, causing tingling, cramps and, if severe, tetany or arrhythmias. Calcium is checked after surgery.
A · The parathyroid glands may have been damaged, lowering calcium
C · Weak breathing at night, so a review for ventilation support
Weak respiratory muscles cause nocturnal hypoventilation and a rise in CO2, leading to morning headaches, disturbed sleep and daytime drowsiness. Non-invasive ventilation can improve symptoms and survival, in line with the patient's wishes.
C · Weak breathing at night, so a review for ventilation support
B · Withdrawal occurs if the drug is stopped suddenly
Physical dependence is a predictable adaptation to regular opioids, causing withdrawal if they stop suddenly, so doses are tapered. Addiction is a behavioural condition with craving, loss of control and continued use despite harm.
B · Withdrawal occurs if the drug is stopped suddenly
B · Phenytoin
Enteral feeds bind phenytoin and lower its levels, risking seizures. Feeds are usually paused around doses, as advised by the pharmacist, and drug levels are monitored.
B · Phenytoin
