1Rumus dan metode

Prioritas & delegasi

Metode

Pengendalian infeksi & keselamatan pasien

Metode

  2. Tambahkan kewaspadaan menurut jalur: kontak (gaun, sarung tangan), droplet (masker bedah), udara (N95, ruang bertekanan negatif).
  3. Jaga barang steril tetap steril: bila ragu, anggap tidak steril; ganti, jangan pernah dilap.

Medikal-bedah: kardiovaskular & respirasi

Metode

  1. Namai masalahnya: kegagalan pompa, arteri tersumbat, irama, bekuan, penyempitan jalan napas, infeksi, atau masalah alat.
  2. Periksa tanda bahaya lebih dulu: hipoksia, hipotensi, nyeri dada, kebingungan baru, anggota gerak yang dingin tanpa denyut.

Medikal-bedah: saraf, endokrin & ginjal

Metode

Medikal-bedah: saluran cerna, perioperatif & onkologi

Metode

Jawaban

A · Orang yang benar
2.Prioritas & delegasi

Jawaban: A · Remove the nurse from patient care and report it according to policy

Mengapa ini jawabannya

A nurse who appears impaired must not care for patients. The charge nurse removes them from duty and follows policy for reporting; support, such as an assistance program, can follow.

Jawaban

A · Remove the nurse from patient care and report it according to policy
3.Pengendalian infeksi & keselamatan pasien

Jawaban: B · A low night light that lights the path to the bathroom

Mengapa ini jawabannya

A night light lets the patient see obstacles and the way to the bathroom without disturbing sleep. Many falls happen in the dark on the way to the toilet.

Jawaban

B · A low night light that lights the path to the bathroom
4.Pengendalian infeksi & keselamatan pasien

Jawaban: A · Keep one up-to-date list of all medicines and use one pharmacy where possible

Mengapa ini jawabannya

A single complete medication list and one pharmacy allow interactions and duplicates to be detected. The list, including over-the-counter drugs and supplements, is shown at every visit.

Jawaban

A · Keep one up-to-date list of all medicines and use one pharmacy where possible
5.Nilai laboratorium & elektrolit

Jawaban: A · Blood glucose, because hypoglycemia can occur

Mengapa ini jawabannya

Insulin shifts potassium into cells but also lowers glucose, and hypoglycemia can occur hours later, especially in kidney failure. Glucose is checked frequently, and potassium is rechecked, as the effect lasts only a few hours.Diperiksa dengan: High potassium level · MedlinePlus (NIH) (2025)

Jawaban

A · Blood glucose, because hypoglycemia can occur
6.Nilai laboratorium & elektrolit

Jawaban: C · Post-streptococcal glomerulonephritis

Mengapa ini jawabannya

Red cell casts show bleeding from the glomeruli, which is typical of glomerulonephritis. After a streptococcal infection, immune complexes inflame the glomeruli, causing hematuria, proteinuria, edema and hypertension. Cystitis and stones do not cause red cell casts.Diperiksa dengan: Strep A Test · MedlinePlus (NIH) (2024)

Jawaban

C · Post-streptococcal glomerulonephritis
7.Keperawatan klinis

Jawaban: C · 0,5 × 109/L

Mengapa ini jawabannya

Neutropenic sepsis is defined by a neutrophil count of 0,5 × 109/L or lower with fever or other signs of sepsis. It needs empirical antibiotics within one hour.

Jawaban

C · 0,5 × 109/L
8.Keperawatan klinis

Jawaban: B · Strict hand hygiene by everyone who has contact with the patient

Mengapa ini jawabannya

Hand hygiene is the single most effective measure to protect neutropenic patients. Rectal procedures and IM injections are avoided because they can cause mucosal or tissue injury and bleeding.

Jawaban

B · Strict hand hygiene by everyone who has contact with the patient
9.Medikal-bedah: kardiovaskular & respirasi

Jawaban: A · Reassess heart rate, blood pressure, urine output and lung sounds

Mengapa ini jawabannya

A fluid challenge is followed by reassessment of perfusion (heart rate, BP, capillary refill, mental state, urine output) and signs of overload such as crackles, raised JVP or worsening breathlessness. The findings are reported to decide on further fluid, vasopressors or escalation.

Jawaban

A · Reassess heart rate, blood pressure, urine output and lung sounds
10.Medikal-bedah: kardiovaskular & respirasi

Jawaban: B · Sternal wound infection with an unstable sternum

Mengapa ini jawabannya

Deep sternal wound infection and sternal dehiscence are serious complications, more common with diabetes, obesity and COPD. Signs include fever, wound discharge, redness and an unstable or clicking sternum. Urgent surgical review, wound swabs, blood cultures and antibiotics are needed.

Jawaban

B · Sternal wound infection with an unstable sternum
11.Medikal-bedah: saraf, endokrin & ginjal

Jawaban: A · The feed can reduce phenytoin absorption, so the feed is often paused around the dose

Mengapa ini jawabannya

Enteral feeds can bind phenytoin and greatly reduce how much is absorbed. Local protocols usually pause the feed before and after the dose and flush the tube. Drug levels are monitored.

Jawaban

A · The feed can reduce phenytoin absorption, so the feed is often paused around the dose
12.Medikal-bedah: saraf, endokrin & ginjal

Jawaban: B · A subarachnoid haemorrhage from a brain aneurysm

Jawaban

B · A subarachnoid haemorrhage from a brain aneurysm
13.Medikal-bedah: saluran cerna, perioperatif & onkologi

Jawaban: D · Pulmonary fibrosis

Mengapa ini jawabannya

Bleomycin can cause pneumonitis and pulmonary fibrosis, especially at higher cumulative doses, in older patients and with high oxygen concentrations. Breathlessness or a new cough must be reported.

Jawaban

D · Pulmonary fibrosis
14.Medikal-bedah: saluran cerna, perioperatif & onkologi

Jawaban: A · 500 mL/h

Mengapa ini jawabannya

Parkland gives 4 × 60 × 25 = 6000 mL in 24 hours, with 3000 mL due in the first 8 hours from the time of injury. With 6 hours left, 3000 ÷ 6 = 500 mL/h. Dividing by 8 (375 mL/h) ignores the delay. The rate is then adjusted to urine output.

Jawaban

A · 500 mL/h
