1Formule e metodi

Controllo delle infezioni e sicurezza del paziente

Metodo

  1. Parti dalle precauzioni standard per ogni paziente: igiene delle mani, DPI per l’esposizione prevista, taglienti in sicurezza.
  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  3. Mantieni sterili i materiali sterili: nel dubbio non è sterile; sostituisci, non pulire mai.
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.
  5. Verifica i diritti della somministrazione dei farmaci, due identificatori e le ulteriori salvaguardie per i farmaci ad alto rischio; segnala errori e quasi-errori.

2.Controllo delle infezioni e sicurezza del pazienteDifficile

An 84-year-old resident with an indwelling catheter suddenly becomes confused but has no fever. What should the nurse consider?

  1. AThe confusion is part of normal aging at this age and needs no further action
  2. BA sedative is needed to keep the resident calm
  3. CInfection can be ruled out because there is no fever
  4. DAn infection may present as confusion in older adults, so assess further and report
3.Fondamenti di infermieristicaMedia

A patient's blood pressure is much higher than in all previous readings. What is the best first step?

  1. ARecheck it, making sure the technique and cuff size are correct
  2. BRecord it and give an extra dose of the blood pressure tablet
  3. CIgnore it, as it must be a machine error

  1. APlasmodium ovale
  2. BPlasmodium vivax
  3. CPlasmodium falciparum

3Soluzioni

1.Controllo delle infezioni e sicurezza del paziente

Risposta: C · Refuse, keep the baby with the mother and alert security

Perché è questa la risposta

Infants are only handed to staff with proper identification, as set out in the infant security policy. Suspicious behavior is reported to security at once, and parents are taught to do the same.

Risposta

C · Refuse, keep the baby with the mother and alert security
2.Controllo delle infezioni e sicurezza del paziente

Risposta: D · An infection may present as confusion in older adults, so assess further and report

Perché è questa la risposta

In older adults, infections such as urinary tract infection may present with new confusion, falls or reduced appetite rather than fever. Sudden confusion is assessed and reported, not dismissed as aging.

Risposta

D · An infection may present as confusion in older adults, so assess further and report
3.Fondamenti di infermieristica

Risposta: A · Recheck it, making sure the technique and cuff size are correct

Perché è questa la risposta

Unexpected findings are validated before action. If the repeat reading is still high, the nurse assesses for symptoms and reports it.

Risposta

A · Recheck it, making sure the technique and cuff size are correct
4.Fondamenti di infermieristica

Risposta: B · Explore the difference gently and assess the pain further

Perché è questa la risposta

When cues conflict, the nurse looks further. Patients may play pain down for many reasons, such as not wanting to bother staff or fearing side effects.

Risposta

B · Explore the difference gently and assess the pain further
5.Microbiologia

Risposta: C · IgM

Risposta

C · Plasmodium falciparum
