1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: B · Day 1 after bowel surgery, new chest pain and heart rate 110/min

Perché è questa la risposta

New chest pain and tachycardia after surgery could be a myocardial infarction or pulmonary embolism. This patient needs immediate assessment; the others have routine needs.

Risposta

B · Day 1 after bowel surgery, new chest pain and heart rate 110/min
2.Priorità e delega

Risposta: B · Respiratory muscle fatigue with possible impending respiratory failure

Perché è questa la risposta

Early in an asthma attack, fast breathing lowers the PaCO₂. A rising, “normal” value in a tiring patient means they can no longer move enough air, which is a warning sign of respiratory failure.

Risposta

B · Respiratory muscle fatigue with possible impending respiratory failure
3.Controllo delle infezioni e sicurezza del paziente

Risposta: D · A quiet “no interruption” zone or signal while preparing medications

Perché è questa la risposta

Interruptions during medication preparation are linked to more errors. Many units use quiet zones, vests or signs to show that the nurse should not be interrupted.

Risposta

D · A quiet “no interruption” zone or signal while preparing medications
4.Controllo delle infezioni e sicurezza del paziente

Risposta: A · A combined approach with education, hand rub at every bedside, regular audits and feedback

Perché è questa la risposta

Multimodal strategies, such as easy access to hand rub at the point of care, training, reminders, audits with feedback and leadership support, improve hand hygiene more than single actions.

Risposta

A · A combined approach with education, hand rub at every bedside, regular audits and feedback
5.Valori di laboratorio ed elettroliti

Risposta: A · The drug can mask warning signs of hypoglycemia

Perché è questa la risposta

Beta-blockers, especially nonselective ones, blunt adrenergic warning signs of hypoglycemia such as tremor and tachycardia, and they can delay recovery. Sweating usually remains. Glucose checks help detect lows that the patient may not feel.Verificato con: Low blood sugar · MedlinePlus (NIH) (2026)

Risposta

A · The drug can mask warning signs of hypoglycemia
6.Valori di laboratorio ed elettroliti

Risposta: D · Anemia of chronic disease (inflammation)

Perché è questa la risposta

In chronic inflammation, iron is trapped in stores, so serum iron is low but ferritin is normal or high and TIBC is low. In iron deficiency, ferritin is low and TIBC is high. Iron supplements help iron deficiency much more than anemia of chronic disease.Verificato con: Ferritin Blood Test · MedlinePlus (NIH) (2025); Serum iron test · MedlinePlus (NIH) (2026)

Risposta

D · Anemia of chronic disease (inflammation)
7.Infermieristica clinica

Risposta: C · Dolore sproporzionato rispetto alla lesione, soprattutto all'estensione passiva

Risposta

C · Dolore sproporzionato rispetto alla lesione, soprattutto all'estensione passiva
8.Infermieristica clinica

Risposta: D · Malignant hyperthermia

Perché è questa la risposta

Malignant hyperthermia is a hypermetabolic reaction triggered by volatile anaesthetics or suxamethonium (succinylcholine) in susceptible people. It is treated urgently with dantrolene, cooling and stopping the trigger.

Risposta

D · Malignant hyperthermia
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: C · Keep a diary of symptoms and activities with the times

Perché è questa la risposta

A Holter monitor records the ECG continuously during normal daily life. A diary of symptoms and activities lets the reviewer match symptoms such as palpitations or dizziness to the rhythm. The device is kept dry unless it is designed to be waterproof.

Risposta

C · Keep a diary of symptoms and activities with the times
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: C · Cover it with a vented chest seal or a dressing taped on three sides

Perché è questa la risposta

An open (sucking) chest wound lets air enter the pleural space. A vented chest seal or a dressing taped on three sides acts as a flap valve, letting air out but not in. A fully sealed dressing can cause a tension pneumothorax. A chest drain is placed away from the wound.

Risposta

C · Cover it with a vented chest seal or a dressing taped on three sides
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: B · Paralysed leg muscles no longer pump blood back to the heart

Perché è questa la risposta

Paralysis removes the calf muscle pump, and immobility causes venous stasis. Trauma also makes the blood more likely to clot. Prophylaxis usually combines anticoagulation and mechanical methods when safe.

Risposta

B · Paralysed leg muscles no longer pump blood back to the heart
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: D · Central cord syndrome

Perché è questa la risposta

Central cord syndrome often follows hyperextension of an arthritic neck. The central fibres, which serve the arms, are affected most, so the arms are weaker than the legs. It is the most common incomplete cord syndrome.

Risposta

D · Central cord syndrome
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta

A · Due o tre evacuazioni morbide al giorno
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: C · Circumferential chest burns, with breathing becoming shallow and laboured

Perché è questa la risposta

Circumferential full-thickness burns of the chest form rigid eschar that restricts chest expansion and can cause respiratory failure. This patient needs immediate breathing assessment, senior help and possibly urgent escharotomy. The others need care but are not immediately life-threatening.

Risposta

C · Circumferential chest burns, with breathing becoming shallow and laboured
