1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: B · That the dose is correct for the child’s weight

Perché è questa la risposta

Pediatric doses are usually calculated by weight, and errors can cause serious harm. The nurse checks the dose against the child’s weight and the safe range before giving it.

Risposta

B · That the dose is correct for the child’s weight
2.Priorità e delega

Risposta: B · Dantrolene

Perché è questa la risposta

Malignant hyperthermia is a reaction to some anesthetic agents that causes hypermetabolism. The triggering drug is stopped and dantrolene is given, with active cooling and support.

Risposta

B · Dantrolene
3.Controllo delle infezioni e sicurezza del paziente

Risposta: D · Only in an urgent situation where delay would harm the patient, per policy

Perché è questa la risposta

Pharmacist review checks for allergies, interactions and dosing errors. Overrides are limited to true emergencies and drugs on an approved list, because bypassing review has led to serious errors.

Risposta

D · Only in an urgent situation where delay would harm the patient, per policy
4.Controllo delle infezioni e sicurezza del paziente

Risposta: B · Airborne and contact precautions until all lesions are crusted

Perché è questa la risposta

Lesions over several areas on both sides mean disseminated zoster, which can spread through the air like chickenpox. Airborne and contact precautions are used until all lesions are dry and crusted. Localized zoster in an immunocompromised patient also needs airborne and contact precautions, but only until dissemination is ruled out.

Risposta

B · Airborne and contact precautions until all lesions are crusted
5.Valori di laboratorio ed elettroliti

Risposta: C · Absolute neutrophil count

Perché è questa la risposta

Clozapine can cause severe neutropenia (agranulocytosis), so the ANC is checked before starting and regularly during treatment. A sore throat, fever or other sign of infection must be reported at once.

Risposta

C · Absolute neutrophil count
6.Valori di laboratorio ed elettroliti

Risposta: D · Ferritin and transferrin saturation

Perché è questa la risposta

Iron deficiency is the most common reason for a poor response to erythropoiesis-stimulating agents, because the marrow needs iron to make hemoglobin. Ferritin and transferrin saturation are checked, and IV iron is often given in dialysis patients. Infection and inflammation can also blunt the response.Verificato con: Anemia of Chronic Kidney Disease · StatPearls (NCBI Bookshelf) (2024); Erythropoietin Stimulating Agents · StatPearls (NCBI Bookshelf) (2024)

Risposta

D · Ferritin and transferrin saturation
7.Infermieristica clinica

Risposta: D · Frequenza cardiaca e pressione arteriosa

Risposta

D · Frequenza cardiaca e pressione arteriosa
8.Infermieristica clinica

Risposta: C · The cold protects the brain, so recovery may be possible after long resuscitation

Perché è questa la risposta

Hypothermia reduces the brain's oxygen needs, and good recoveries have occurred after long resuscitation. Death is usually not confirmed until the patient has been rewarmed, unless injuries are clearly not survivable.

Risposta

C · The cold protects the brain, so recovery may be possible after long resuscitation
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: B · No more than half the internal diameter of the tube

Perché è questa la risposta

A catheter no larger than half the internal diameter of the tube lets air enter around it, reducing negative pressure in the lungs, atelectasis and hypoxaemia. Catheter size is often calculated from the tube size.

Risposta

B · No more than half the internal diameter of the tube
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: A · Stop smoking, control BP and seek help at once for sudden back pain

Perché è questa la risposta

Small aneurysms are usually monitored with regular ultrasound, with repair considered when they reach a threshold size or grow fast. Stopping smoking slows growth, and BP and cardiovascular risk are managed. Sudden abdominal or back pain may mean rupture and needs emergency care.

Risposta

A · Stop smoking, control BP and seek help at once for sudden back pain
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: C · No antibiotics, as long as there are no symptoms

Perché è questa la risposta

Nearly all long-term catheters become colonised. Treating asymptomatic bacteriuria does not improve outcomes and promotes resistance. Antibiotics are reserved for symptomatic infection.

Risposta

C · No antibiotics, as long as there are no symptoms
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: C · Capillary glucose, for hypoglycaemia

Perché è questa la risposta

Hypoglycaemia is a common complication of insulin–glucose treatment and can occur several hours later, especially in kidney failure. Glucose is checked regularly according to protocol, and potassium is rechecked.

Risposta

C · Capillary glucose, for hypoglycaemia
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: A · Treatment before the main treatment, often to shrink the tumour

Perché è questa la risposta

Neoadjuvant chemotherapy or radiotherapy is given before surgery to shrink the tumour, which may make surgery easier or allow less extensive surgery. Adjuvant therapy follows surgery.

Risposta

A · Treatment before the main treatment, often to shrink the tumour
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: B · Toxic megacolon

Perché è questa la risposta

Toxic megacolon is dilatation of the inflamed colon with systemic toxicity and a high risk of perforation. Antimotility drugs and opioids are avoided, an abdominal X-ray is done and the surgical team is involved.

Risposta

B · Toxic megacolon
