1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: D · Contact the prescriber to clarify the order before giving the drug

Perché è questa la risposta

An unclear order must be clarified with the prescriber before the medication is given. Guessing or ignoring the order can harm the patient.

Risposta

D · Contact the prescriber to clarify the order before giving the drug
2.Priorità e delega

Risposta: C · Wear a gown and gloves for contact with the patient or the room, and use dedicated equipment

Perché è questa la risposta

MRSA spreads through direct contact and contaminated surfaces. Gown and gloves, hand hygiene and dedicated equipment prevent transmission; a respirator is not needed for contact precautions.

Risposta

C · Wear a gown and gloves for contact with the patient or the room, and use dedicated equipment
3.Controllo delle infezioni e sicurezza del paziente

Risposta: D · Move the walker forward, then step into it

Perché è questa la risposta

The patient moves the walker forward, sets it down with all legs on the floor and then steps into it. To stand, they push up from the chair arms, because a walker can tip if used to pull up.

Risposta

D · Move the walker forward, then step into it
4.Controllo delle infezioni e sicurezza del paziente

Risposta: C · A combined approach with education, hand rub at every bedside, regular audits and feedback

Perché è questa la risposta

Multimodal strategies, such as easy access to hand rub at the point of care, training, reminders, audits with feedback and leadership support, improve hand hygiene more than single actions.

Risposta

C · A combined approach with education, hand rub at every bedside, regular audits and feedback
5.Valori di laboratorio ed elettroliti

Risposta: C · Biotin can distort some immunoassays, such as troponin or TSH

Perché è questa la risposta

Many immunoassays use biotin-streptavidin binding, and high blood biotin levels can give falsely high or low results, for example a falsely low troponin or abnormal thyroid tests. The laboratory and prescriber should know about biotin use.Verificato con: Troponin Test · MedlinePlus (NIH) (2023); Biotin Interference with Troponin Lab Tests - Assays Subject to Biotin Interference · U.S. FDA (2022)

Risposta

C · Biotin can distort some immunoassays, such as troponin or TSH
6.Valori di laboratorio ed elettroliti

Risposta: B · Large-volume IV fluid replacement

Perché è questa la risposta

In HHS the fluid deficit is often very large, so IV fluid replacement is the first priority; it also lowers the glucose. Insulin follows as prescribed, and potassium, sodium and mental status are monitored closely.Verificato con: Diabetic hyperglycemic hyperosmolar syndrome · MedlinePlus (NIH) (2026)

Risposta

B · Large-volume IV fluid replacement
7.Infermieristica clinica

Risposta: B · Offload the heels so they do not touch the mattress

Perché è questa la risposta

Heels are a common pressure ulcer site. Offloading devices or pillows under the calves keep the heels free of the mattress; massage over bony prominences can damage tissue.

Risposta

B · Offload the heels so they do not touch the mattress
8.Infermieristica clinica

9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: D · Sitting upright and leaning forward over a table

Perché è questa la risposta

Sitting upright and leaning forward on a bedside table widens the intercostal spaces at the back and lets the fluid settle at the base. If the patient cannot sit, lying on the unaffected side may be used.

Risposta

D · Sitting upright and leaning forward over a table
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: C · A patient with heart failure who is newly confused, with SpO₂ 89%

Perché è questa la risposta

New confusion with hypoxaemia suggests acute deterioration and needs immediate assessment. A COPD patient within their usual target range, a request for tea and an improving DVT are not urgent. Delegated observations must be reported and acted on promptly.

Risposta

C · A patient with heart failure who is newly confused, with SpO₂ 89%
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: C · Carbamazepine can make the pill less effective, so other contraception is needed

Perché è questa la risposta

Carbamazepine is a strong liver enzyme inducer, so it lowers hormone levels from the pill and can lead to an unplanned pregnancy. Contraception that is not affected, such as an IUD, is usually advised.

Risposta

C · Carbamazepine can make the pill less effective, so other contraception is needed
12.Area medico-chirurgica: neurologica, endocrina e renale

13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: D · Awake, with a clear airway, stable observations and controlled pain

Perché è questa la risposta

Discharge criteria include a protected airway, adequate breathing and oxygen saturation, stable vital signs, acceptable pain and nausea control, and no active bleeding, as set by local recovery criteria.

Risposta

D · Awake, with a clear airway, stable observations and controlled pain
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: B · A high spinal block

Perché è questa la risposta

If local anaesthetic spreads upwards, it can affect the arms, breathing muscles and cardiac nerves, causing hypotension and bradycardia. It is an emergency; help is called, and breathing and circulation are supported.

Risposta

B · A high spinal block
